摘要 目的探讨使用CT及MRI技术在脑弥漫性轴索损伤诊断中的作用。方法回顾性分析该院收治的26例脑弥漫性轴索损伤患者的临床资料,均分别使用了CT及MRI技术进行诊断,探讨两种检查对病灶的敏感度差异。结果 CT发现病灶数69个,MRI发现病灶数210个,MRI发现病灶数显著多于CT(P<0.05);MRI检出脑内非出血病灶35个,显著高于CT检查的9个(P<0.05);对脑深部区域的检查敏感度,MRI为18.1%(38/210),高于CT检查的8.7%(6/69),两者比较差异有统计学意义(P<0.05);对于大脑皮层下区域的检查敏感度,两者比较差异无统计学意义(P>0.05)。结论在诊断脑弥漫性轴索损伤中,MRI检查敏感,尤其对脑内非出血病灶和深部病灶敏感度高,较CT检查优势更明显。 Objective To investigate the functions of CT and MRI technology in the diagnosis of diffuse axonal injury. Methods The clinical data of 26 patients with diffuse axonal injury admitted in our hospital were retrospectively analyzed. All patients were given CT and MRI technology diagnosis. The sensitivities of these two technologies for lesions were compared. Results 69 lesions were found in CT. 210 lesions were found in MRI. The number of lesions found by MRI were much more than those by CT (P〈0.05); 35 brain non- hemorrhagic lesions were detected in MRI, the number was much more than 9 detected by CT (P〈0.05); The inspection sensitivity of deep brain region of MRI was 18.1% (38/210), which was significantly higher than 8.7% (6/69) of CT, the difference was statistically significant (P〈O.05). For the inspection sensitivity of subcortical regions, the difference between CT and MRI had no statistically significant difference (P〉O.05) Conclusion In the diagnosis of diffuse axonal injury, MRI is highly sensitive especially for non- hemorrhagic brain lesions and deep lesions with more advantages than CT.
