宫颈组织高危型HPV(16/18型)感染与宫颈癌及癌前病变发生的相关性分析

Analysis of the Correlation of High-risk HPV(16/18 type)of Cervical Tissue Infection with Cervical Cancer and Precancerous Lesions

目的:通过观察发生宫颈病变的妇女的人乳头瘤病毒(HPV)感染情况,比较分析宫颈组织高危型HPV(16/18型)感染与宫颈癌及癌前病变发生的相关性。方法:选取在我院妇科就诊的怀疑有宫颈病变的患者656人,对所有患者进行病理活检及专业的病理诊断,并进行HPV-16和HOV-18的基因学检测,对结果进行分析。结果:与无宫颈病变组比较,CINⅠ组、CINⅡ/Ⅲ组、宫颈癌组高危型HPV DNA阳性率明显增高(P<0.05);与CINⅠ组比较,CINⅡ/Ⅲ组、宫颈癌组的高危型HPV DNA阳性率明显增高(P<0.05);感染HPV-16和HPV-18后,发生宫颈癌的危险度均较高,OR分别为48.85和26.00,发生CINⅡ/Ⅲ的危险度也较高,OR分别为11.59和19.36。结论:宫颈组织高危型HPV(16/18型)感染与宫颈癌及癌前病变发生存在一定的相关关系,建议临床治疗宫颈癌前病变时注意HPV感染,患者应注意自我保护,避免HPV的感染。

Objective To compare and analyze the correlation of high-risk HPV（16/18 type） of cervical tissue infection with cervical cancer and precancerous lesions by observing the HPV （human papilloma virus） infection of women with cervical lesions. Methods 656 patients suspected with cervical lesions treated in gynecology of our hospital were selected as the study objects. Biopsy and professional pathological diagnosis were adopted; HPV-16 and HOV-18 were detectedin genetics, then the results were analyzed. Results Compared to group without cervical cancer, the positive rates of high risk HPV DNA of CIN Ⅰ group, CIN Ⅱ/Ⅲ group and cervical cancer group were increased significantly （P〈0.05）; and the positive rates of high risk HPV DNA of CINⅡ/Ⅲ group and cervical cancer group were increased significantly compared to CIN Ⅰ group （P〈0.05）; the risk degree was higher after infecting with HPV-16 and HPV-18, OR were 48.85 and 26.00 respectively; the risk degree of CINⅡ/Ⅲ was higher too, OR were 11.59 and 19.36 respectively. Conclusion High-risk HPV （16/18 type） of cervical tissue infection has certain correlation with cervical cancer and precancerous lesions. Attention should be paid to HPV infection and self-protection, so as to avoid HPV infection.
