摘要
目的探讨基于DWI产生的ADC值与宫颈癌的组织病理学特征的相关性。方法对65例宫颈癌患者的资料进行回顾性分析,根据手术病理结果分析肿瘤分化程度、类型、淋巴结状态(0,1,2,3),以及有无微血管或微淋巴管浸润。所有患者均在术前采用3.0 T MRI常规进行MRI及DWI扫描。结果 ADC值在低分化宫颈癌(G3)与高中分化宫颈癌(G1/2)之间,在宫颈鳞癌与腺癌之间差异有统计学意义。ADC值在不同淋巴结状态(0,1,2,3)之间及有无微脉管浸润之间差异无统计学意义。结论 ADC值可提示宫颈癌分化程度及类型,为制订无创治疗方案提供参考。
Objective To evaluate the potential correlation between apparent diffusion coefficient (ADC) which derived from diffusion-weighted imaging (DW1) and the histopathology feature of cervical cancer. Methods We retrospectively analysed 65 patients with cervical carcer, and divided these patients according the following postoperative histopathological characteristics:(1) pathologic grades of cervical cancer (G1, G2, G3); (2) type of cancer: squamous carcinoma or adenocarcinoma; (3) lymph node status (0,1,2,3); (4) microvascular invasion statue: yes or no. All patients underwent diffusion-weighted imaging (DWI) in addition to routine MR imaging with a 3.0T MRI. Results There was a statistically significant difference between the ADCs of well/moderately differentiated tumors (G1/2) and poorly differentiated tumors (G3), and between the ADCs of squamous carcinoma and adenocarcinoma. ADCs was not associated with lymph node status and microvascular invasion. Conclusion ADCs can be used to indicate degree and histologic type of cervical cancer, and thus noninvasively provide valuable information for the planning of therapeutic regimen.
