颅底脑膜瘤的手术治疗

目的 探讨颅底脑膜瘤手术取得成功的关键。方法 分析1996.1～1999.12收治的20例颅底脑膜瘤的临床资料及手术方法。结果 SimpsonⅠ级切除8例、Ⅱ级切除10例、Ⅲ级切除2例。全组无手术死亡 ,术后1例发生偏瘫、失语 ,余均顺利恢复。结论 术中控制出血、在蛛网膜间隙内操作是切除颅底脑膜瘤取得成功的关键 ;肿瘤侵犯血管时不宜强求全切 。

Objective To analyze key points in surgical treatment of meningioma on base of skull. Methods Clinical data and operation methods of meningioma on base of skull were reviewed (n=20, 1996.1-1999.12). Results 8 cases of Simpson I, 10 cases of Simpson II and 2 cases of Simpson III were operated without surgical death, but one case was hemiplegia and agnosia after oper-ation. Conclusion Control of hemorrhage and operating in arachnoid space were the key points. To meningioma affecting vessel, it was inadvisable to excise radically, and radiotherapy could be combined.
