妊娠期糖尿病IADPSG诊断标准的临产病例分析

Analysis of IADPSG diagnosis standard of diabetes in pregnancy parturient cases

目的分析国际妊娠与糖尿病研究组织(IADPSG)妊娠期糖尿病(GDM)诊断标准对母儿结局的影响。方法回顾性分析采用NDDG诊断标准的2011年住院分娩的产妇116例(NDDG组)和采用IADPSG诊断标准的2012年住院分娩产妇805例(IADPSG组)的母儿结局,从孕妇并发症、分娩方式及新生儿结局三分面对比分析。结果两组孕妇羊水过多、胎膜早破、产后出血发生率无统计学差异(P>0.05),两组羊水过少、剖宫产率、巨大儿、早产儿、新生儿畸形、新生儿高胆红素血症发生率存在统计学差异(P<0.05)。结论采用IADPSG诊断标准GDM能有效降低母儿不良结局的发生率。

Objective To study the effect of the International Association of Diabetes and Pregnancy Study Group diagnostic crite- ria to the outcome of gestational diabetes mellitus （GDM）. Methods The maternal and fetal outcomes of 116 cases of maternal hos- pital delivery （group NDDG, according to NDDG diagnostic criteria ） in 2011 and 805 cases of maternal hospital delivery （group I- ADPSG,using the IADPSG diagnostic criteria）were retrospectively analysed. The maternal and fetal outcomes were comparative analysed from three aspects of pregnant complication,mode of delivery and neonatal outcome.Results There were not significant difference of pregnant women, polyhydramnios, premature rupture of membranes, postpartum hemorrhage rate between two groups （P〉0.05）.Two groups of oligohydramnios, fetal macrosomia, cesarean section, premature infant, neonatal malformations, the inci- dence of neonatal hyperbilirubinemia were statistically significant （P〈0.05）.Conclusion The diagnostic criteria of IADPSG GDM can effectively reduce the incidence of adverse outcomes of mothers and infants.
