摘要
目的:研究早期肠内营养对胰十二指肠切除术后感染的影响,以此为临床防控感染提供一定的参考。方法:选取行胰十二指肠切除术的患者96例,根据数字随机表法分为实验组(48例)和对照组(48例)。实验组行肠内营养支持,对照组行肠外营养支持,观察两组患者感染情况及肝功能水平。结果:两组患者肺部感染率、切口感染率比较无统计学意义;但实验组腹腔内感染率和腹腔外感染率均明显低于对照组,差异有统计学意义。对照组患者(1-3)-β-D葡聚糖浓度及内毒素术后第8天显著高于对照组。结论:胰十二指肠切除术后早期肠内营养支持可有效改善患者营养状态,保持患者肝功正常,减少腹腔内外感染的发生,对于临床防控感染有一定的积极作用。
Objective:To study the effect of nutrition on intestinal infection after pancreaticoduodenectomy early,in order to provide a reference for clinical prevention and control of infection.Methods:The patients underwent pancreaticoduodenectomy in 96cases,according to the figures were randomly divided into experimental group(48cases)and control group(48cases).Experimental group enteral nutrition;control group underwent parenteral nutritional support.Infection were observed and liver levels.Results: The two groups of patients lung infection,wound infection rate was not statistically significant;,but the experimental group were abdominal infection and infection rates were significantly lower than the control group,the difference was statistically significant. Control patients(1-3)-β-D glucan concentration was significantly higher than the first eight days after endotoxin.Conclusion:Postoperative pancreaticoduodenectomy early enteral nutrition can improve the nutritional status of patients,patients with normal liver function maintained,both inside and outside the abdominal cavity to reduce the incidence of infection,prevention and control of infection for clinical some positive effect.
