踏车运动试验结合核听诊器检查对冠心病诊断价值的探讨

Evaluation for Diagnosis of Coronary Heart Disease by the Exercise Test Combined with the Nuclear Stethoscope Test

本研究对20例冠心病、21例可疑冠心病及21例正常人,同步进行踏车运动试验及核听诊器左心功能测定,以运动前后sT及非sT参数多变量指标结合LVEF测定综合分析,发现QTr或△EF和△QTr或△EF为诊断冠心病的最佳指标并可判断左室功能。

The ergometric bicycle exercise test combined with the left ventricular function test by nuclear stethoscopy in 20 cases of coronary heart disease (CHD), 21 cases of suspected CHD and 21 normal persons were reported. ECOT, ST, QTr, △QTr, △R, △ST, △RST, △Q and LVEF were measured in all the subjects before the tests, during the exercise and immediately at the end of the exercise. The results revealed that if we took the single index of the electrocardiographic changes such as ST, QTr, △QTr or △EF as a criterion for the diagnosis of CHD, there was no significant difference0 among the four indexes (P>0.05), and with a low sensitivity, specificity and correctivity for detecting CHD, but the analysis of multiple variations showed that in both groups of the indexes QTr or △EF and △QTr or △EF, the sensitivity, specificity, correctivity were 100％, 81％ and 90％, respectively. Therefore, these two compound indices might be considered as the best criteria for the diagnosis of CHD.
