Antiarrhythmic effects of propofol on reperfusion induced ventricular arrhythmia in isolated rat hearts

摘要 目的 :实验观察异丙酚对大鼠离体心脏再灌注心律失常的影响。方法 :SD大鼠Langendorff离体心脏灌注后 ,建立冠状动脉左前降支局部心肌缺血和再灌注心律失常模型。分 6组 :对照组 ;异丙酚 1μg/ml组 ;异丙酚3μg/ml组 ;维拉帕米 10ng/ml组 ;异丙酚 1μg/ml加维拉帕米 10ng/ml组 ;异丙酚 3μg/ml加维拉帕米 10ng/ml组。定时测量冠脉流量、心率和连续监测ECG ,以比较各组再灌注期室性期前收缩 (VPB)、心室纤颤 (VF)、室性心动过速 (VT)的发生率以及正常窦性节律 (NSR)的持续时间。结果 :与对照组相比 ,后 4组再灌注期的VF发生率均有非常显著地降低 (P <0 .0 1) ,VF的持续时间显著缩短 (P <0 .0 1) ,NSR的时间明显延长 ,而VT的发生率仅有下降趋势。结论 :3μg/ml的异丙酚与 10ng/ml的维拉帕米相似 。 Objective:To observe the effect of propofol on ventricular arrhythmia in isolated heart of rats induced by reperfusion.Methods:Arrhythmic model in isolated heart of SD rats was created by Lengendorff reperfusion using a solution contaning NaCl 118,KCl 4.7,KH 2PO 4 1.2,MgSO 4 1.2, CaCl 2 2.5,NaHCO 3 25,glucose 11.1,and EDTA 0.125mmol/L.Forty seven isolated heart models were divided into control (Cntrl, n =12),1μg propofol(P1, n =7),3μg propofol(P3, n =9),10 ng verapamil(Ver, n =7),1μg propofol plus 10 ng verapamil(P1+V, n =6),and 3μg propofol plus 10 ng verapamil(P3+V, n =6) groups.All of the left anterior descending coronary artery was clamped 10 min to result in regional ischemia and reperfused 5 min to create reperfusion arrhythmia. During reperfusion time, the incidence of ventricular fibrillation (VF) and ventricular tachycardia (VT) as well as the duration of VF and normal sinus rhythm (NSR) was measured.Results:The incidence of VF and VT during reperfusion time is 9/12 and 8/12 in Cntrl group;4/7 and 5/7 in P1; 0/9 ( P <0.01 vs. Cntrl) and 2/9 in P3;0/7( P <0.01) and 5/7 in Ver; 0/6( P <0.01) and 1/6 in both P1+V and P3+ V.The duration of VF in group Cntrl and P1 is(143±126)s and(146±146)s respectively.In P3,Ver,P1+V and P3+V groups,all the duration of VF is(0±0)s( P < 0.01). The duration of NSR in group Cntrl and P1 is(106±135)s and(144±149)s respectively,in group P3,Ver,P1+V and P3+V is(262±99)s( P <0.01),(272±26)s( P <0.05),(270±74)s( P <0.05) and (295±14)s( P < 0.01) respectively.Conclusion:As compared with the control group, propofol 3 μg/ml similar to verepamil 10 ng/ml, significantly reduced the incidence and duration of reperfusion induced VF.Furthermore,it markedly increased the duration of NSR in the reperfusion time.Nevertheless, propofol 1 μg/ml presented little antiarrhythmic effects.
