基于主题模型的消渴病痹痿症药关系研究

Symptom and medication relationship of Bi-symptom in diabetes mellitus based on topic model

目的基于主题模型对真实世界临床数据分析、挖掘,研究中医治疗消渴病痹痿症状与用药的关系。方法利用临床科研信息共享平台采集系统采集消渴病痹痿患者的症状及用药数据1 073条,对其中同时具备症、药信息的患者316例,运用症状、用药间的LDA主题模型挖掘和分析症药之间的关系。结果消渴病痹痿的主要症状及用药规律:肢体麻木疼痛,加用红花、桃仁、川芎、地龙活血祛瘀通络;口干、乏力、视物模糊,加用丹参、白芍、地黄滋阴养血、祛瘀生新;胸闷,加用葛根、丹参活血化瘀;胸闷、乏力、头晕、心悸,加用麦冬、沙参、太子参益气生津复脉;手足麻木、疼痛、畏寒,加用桂枝、白芍、葛根、细辛、甘草温经脉、通血络;纳差、失眠、便秘,加陈皮、半夏、黄连清热化痰。结论消渴病痹痿人群多,症状、药物相对单一。抓主症、依症状选效药符合中医对症论治特点。

Objective To explore and analyze the chnical data of real word based on topic model, in order to study symptom and medication relationship of Bi-symptom in treatment of diabetes mellitus （DM） in TCM. Methods Using information sharing platform system of clinical research, 1073 pieces of symptoms or medication data of Bi-symptom in DM patients were collected, and 316 cases which had both symptoms and medication information were analyzed with LDA topic model to explore the symptom and medication relationship. Results The main symptom and medication rule： Flos carthami, Femen persicae, Rhizoma Ligustici chuanxiong and Lumbricus were added if there was numbness and pain in the limbs; Radix salviae miltiorrhizae, Radix paeoniae alba and Radix rehmannia recens were added if there was dry month, fatigue and blurred vision; Radix puerariae and Radix salviae miltiorrhizae were added if there was oppression in chest; Radix Ophiopogonis, Adenophora stricta and Radix pseudostellariae were added if there was oppression in chest, fatigue, dizziness and palpitation; Ramulus cinnamomi, Radix paeoniae alba, Radix puerariae, Herba asari and Radix glycyrrhizae were added if there was numbness in the limbs, pain and fear of cold ; Pericarpium citr reticulatae, Rhizoma pinelliae and Rh/zoma coptidis were added if there was poor appetite, insomnia and constipation. Conclusion The number of Bi-symptom in DM patients is considerable with relatively single symptoms and medications. Focusing on the main symptoms and selecting medicine according to symptoms meet the characteristic of syndrome differentiation and treatment in TCM.
