2007-2011年上海市长宁区精神疾病患者康复状况分析

Psychiatric Rehabilitation Status of Patients in Changning District of Shanghai during 2007- 2011

目的 了解2007-2011年上海市长宁区精神疾病患者状况,为疾病防治提供依据.方法 分析5年间患者人口学特征、参与社会及监护状况并计算检出率、治疗率、显好率等指标.结果 患者男女相当,36 ～ 59岁约占56％（x2=169.12,P=o.000）,中学、中专占61.6％（x2=205.66,P=0.000）,未婚接近50％（x2 =7.59,P＜0.05）,差异有统计学意义c服务对象中前3类精神疾病依次为精神分裂症、精神发育迟滞与童年和少年期心理障碍、心境障碍（x2=62.10,P=o.000）,前3位主要监护人依次为父母、配偶、子女（x2=23.07,P＜0.05）,能参与（临时）工作的不足10％,各项工作指标呈现逐年递增态势.结论 重性精神疾病患者是长宁社区精神卫生服务主要对象,建议提高疾病监测能力,推进专业康复指导,加大队伍建设,完善体系功能.

Objective To understand the psychiatric rehabilitation status of patients in Changning district of Shanghai during 2007 - 2011, and provide scientific evidence for prevention and treatment of mental disease. Methods We analyzed demographic characteristics of patients with mental disease, the situation of participating in social and family care during five years, meanwhile calculating working in- dex such as the detection rate, the rate of significant index, the treatment rate and other indicators. Results The proportion of men and women were basically the same, secondary education accounted for 61.6% （ X2 = 205.66, P = 0. 000）, the unmarried ratio was close to 50% （ X2 = 7.59, P 〈 0.05 ） and these differences were statistically significant. The top 3 mental diseases during five years was schizo- phrenia and other psychotic disorders, mental retardation and childhood and adolescent mental disorder, mood disorder（ X2 = 62.10, P = 0. 000）, meanwhile the main guardian were parents, spouses and children, an average of less than 10% to participate in the work or tem- porary work and school（ X2 = 23.07, P 〈 0.05 ） and these indexes had been increasing trend. Conclusion Patients with severe mental disease are the main object of community mental health services at the present stage in Changning district. We propose to improve disease identification and monitoring ability, expand the service coverage, increase the intensity of professional guidance, strengthen the service team construction and finally perfect the system of mental - health service in the community.
