瘢痕子宫再次剖宫产围手术期并发症的临床分析

Clinical Complication Study During Perioperative Cesarean Section Once More for Parturient with Cicatricle Uterus

目的：分析剖宫产后瘢痕子宫再次剖宫产围手术期并发症的发生情况，并探讨降低手术风险的有效措施.方法：对2012年1月~2013年1月在我医院住院分娩的瘢痕子宫患者90例设为观察组，将同期首次剖宫产的非瘢痕子宫的孕妇90例设为对照组，对比两组患者剖宫产时并发症的发生情况.结果：瘢痕子宫再次妊娠分娩方式中再次剖宫产率为82％，其发生盆腹腔粘连率、子宫破裂率、产后出血率、胎盘异常（前置胎盘，胎盘粘连，植入）发生率等并发症均明显高于初次剖宫产患者（P＜0．05）.结论：瘢痕子宫再次剖宫产时围手术期并发症明显增多，需要加强干预，减少并发症发生.

Objective：To analyze complications during perioperative cesarean section once more for partu-rient with cicatricle uterus,and to explore the effective measures to reduce the risk of surgical operation. Methods：90 cases of parturient with cicatricle uterus in our hospital during January 2007 to January 2013 were supposed as observation group,and other 90 cases of parturient without cicatricle uterus at the same time were supposed as control group.The complications during perioperative period of cesarean section were compared.Results：The complication rate of selected cesarean section once more was 82% in observation group,and the rate of adhesion of abdominopelvic cavity,hysterorrhexis,postpartum hemorrhage,placental abnormality （containing placenta praevia、placental adherence and embedding）in observation group are sig-nificantly higher than that of the control group （P＆lt;0.05）.Conclusion：The complication during perioperative period of cesarean section once more for parturient with cicatricle uterus are significantly increased.So,in or-der to prevent the occurrence of complications,special care of those parturient with cicatricle uterus should be took.
