磁共振不同扫描技术在新生儿缺血缺氧性脑病中的应用 被引量:6

摘要 目的:分析磁共振常规序列及DWI、SWI在新生儿缺血缺氧性脑病(HIE)中诊断价值。材料与方法:收集2010-2012年新生儿HIE患者64例,行常规MRI检查及DWI、SWI检查,对常规MRI、DWI、SWI图像进行分析,评价不同扫描技术在HIE中诊断优势。结果:新生儿HIE的cMRI+DWI+SWI多序列组合扫描观察到(1)弥漫性脑水肿18例;(2)脑出血改变31例;(3)脑室周围梗死1例;(4)皮质、皮质下及深部白质异常信号27例。64例新生儿HIE中,cMRI+DWI+SWI共检出53例患儿,104个病灶,cMRI共检出28例阳性病例,检出49个病灶,cMRI+DWI共检出41例,66个病灶。结论:DWI主要反映新生儿HIE早期病理改变,而常规MRI反映亚急性期病变较好。SWI对HIE合并颅内出血的检测较常规MRI要敏感的多,提高了新生儿颅内出血的检出率,cMRI+DWI+SWI多序列组合扫描能全面反映新生儿HIE的病理变化,是检查新生儿HIE的最佳序列组合。 Purpose: Analysis of regular mri sequence and DWI, SWI can in neonatal hypoxic ischemia encephalopathy (HIE) diagnostic value. Materials and Methods: Collected from 2010 to 2012, 64 cases of neonatal HIE patients, routine MRI and DWI, SWI can check, for routine MRI and DWI, SWI can image analysis, evaluation of different scanning technology advantage in HIE diagnosis. Results: Neonatal HIE cMRI + DWI + SWI sequence combination can scan ( 1 ) diffuse brain edema was observed 18 cases; (2) the bleeding in the brain change 31 cases; (3) ventricular infarction around 1 case; (4) cortical, subcortical and deep white matter abnormalities in 27 cases. 64 cases of neonatal HIE, cMRI + DWI + can SWI 53 cases were detected, 104 lesions, cMRI detected 28 cases were positive, check out 49 lesions, cMRI + DWI were detected with 41 cases, can 66 lesions. Conclusion: DWI mainly reflects a newborn HIE can early pathological changes, and conventional MRI better reflect the subacute stage lesions. SWI on HIE combined detection of intracerebral haemorrhage more sensitive than conventional MRI, improve the rate of neonatal intracranial hemorrhage, cMRI + DWI + SWI sequence com- bination can scan can comprehensively reflect the pathological changes of the neonatal HIE, combination is the best sequence to check the neonatal HIE.
