内固定辅助铰链式外固定支架治疗肘部损伤“三联征”的疗效分析 被引量:12

摘要 目的探讨内固定辅助铰链式外固定支架治疗肘部损伤“三联征”的疗效。方法2007年7月至2012年7月采用肘关节内、外侧联合入路切开复位内固定辅助铰链式外固定支架治疗11例肘部损伤“三联征”患者,男10例,女1例;年龄为23~48岁,平均37.2岁;左侧2例,右侧9例;均为闭合性损伤。桡骨头骨折按Mason分类:Ⅱ型3例,Ⅲ型8例。尺骨冠状突骨折按Regan—Morrev分类:I型4例,Ⅱ型6例,Ⅲ型1例。末次随访时测量肘关节屈伸和前臂旋转范围,并根据Mayo肘关节功能评分(MEPS)标准评定疗效。结果11例患者术后获6~15个月(平均9.5个月)随访。所有患者伤口均一期愈合,骨折均在术后3个月左右获临床愈合。末次随访时肘关节平均屈伸活动度为109.2°,前臂旋转活动度平均为118.7°;根据MEPS评分标准评定疗效:优3例,良6例,可2例。均未出现异位骨化及关节僵硬。结论内固定辅助铰链式外固定支架治疗肘部损伤“三联征”可重建肘关节解剖结构、恢复其稳定性、早期行功能锻炼,最大限度地恢复肘关节功能。
