椎体成形术与椎体后凸成形术治疗椎体压缩性骨折疗效的Meta分析 被引量:52

Percutaneous vertebroplasty vs percutaneous kyphoplasty for treating osteoporotic vertebral compression fractures: a Meta analysis

摘要 [目的]系统评价椎体成形术与椎体后凸成形术治疗椎体压缩性骨折的有效性和安全性。[方法]计算机检索数据库,手工检索骨科相关杂志,纳入关于比较椎体成形术与椎体后凸成形术治疗椎体压缩性骨折的研究并进行Meta分析。[结果]纳入15篇研究共1 082例椎体,其中PVP组563例,PKP组519例,进行Meta分析。结果显示椎体成形术与椎体后凸成形术治疗椎体压缩性骨折相比较,VAS及骨水泥渗漏率无明显统计学差异。椎体后凸成形术对Cobb角的矫正度数大于椎体成形术,差异具有统计学意义。[结论]椎体成形术与椎体后凸成形术治疗椎体压缩性骨折在缓解疼痛和骨水泥渗漏上无明显的区别。在矫正Cobb角度上,椎体后凸成形术较椎体成形术具有显著的优越性。 [ Objective ] To compare the efficacy and safety of percutaneous vertebroplasty with that of percutaneous ky- phoplasty in the treatment of osteoporotic vertebral compression fractures. [ Method ] We performed a systematic search of Med- line, EMBASE, and Cochrane Central, as well as some related journals to collect those controlled trials. The RevMan 5.1 soft- ware was used for meta- analysis. [ Result] In total, 15 studies met the inclusion criteria, including a total of 1082 cases. Meta -analysis indicated that the postoperative Cobb~ angles were significantly different between the 2 procedures,but no differences were noted for the visual analogue scale score at the lower back and cement leakage rate. [ Conclusion] Both procedures are ef- fective and safe with regard to pain relief and cement leakage for the treatment of osteoporotic vertebral compression fractures. However,percutaneous kyphoplasty is more effective than percutaneous vertebroplasty in terms of the correction of kyphosis. Thus, we observed that kyphoplasty was biomechanically superior to vertebroplasty.
