子宫全切除术对女性生殖激素、性功能及更年期综合症的影响研究

Effect of total hysterectomy on reproductive hormone and sexual function

目的:分析子宫全切除术对女性生殖激素、性功能及更年期综合症的影响。方法:选择在本院接受子宫全切伴双侧卵巢保留手术的患者作为观察组对象,另取同期在本院进行体检的一般情况类似的妇女作为对照组对象,比较两组对象的激素水平、性生活情况、更年期症状发生情况等差异。结果:观察组患者接受治疗后半年、1年的血清FSH(10.76±3.21、11.74±3.84)mIU/mL水平明显高于对照组,LH(5.76±1.32、5.32±1.05)mIU/mL及E2(37.42±8.67、36.13±7.23)pg/mL水平明显低于对照组;性生活满意率(30.88%)、性欲下降(25%)、性兴奋缺乏(17%)、性交痛(11.76%)明显低于对照组;潮热出汗(54.41%)、焦躁(33.82%)、失眠(39.71%)、疲劳乏力(30.88%)、心悸(23.53%)等更年期症状明显高于对照组(P<0.05)。结论:子宫全切术可以降低患者的卵巢功能,影响其正常激素水平,降低性欲、提高围绝经期综合征的发生率,降低其生活质量。

Objectives： To analyze the effect of total hysterectomy on women reproductive hormone,sexual function and climacteric syndrome.Method： Patients receiving hysterectomy with bilateral ovarian conservation were selected as the observation group.The control group include the women who had similar condition.The hormone levels,sexual life quality and menopausal symptoms were compared.Results： The FSH level of patients receiving treatment after 6 months and 1 year was significantly higher than that of the control group （10.76 ± 3.21 ;11.74 ±3.84 mIU/mL） ; LH （5.76±1.32; 5.32±1.05 mIU/mL） and E2 （37.42 ±8.67; 36.13-±7.23pg/mL） levels were significandy lower than that of the control group.The sexual life satisfaction （30.88％）,decreased libido （25％）,sexual organsm （17％） and sexual intercourse pain（11.76％） were significantly lower than that of the control group; while sweating （54.41％）,anxiety （54.41％）,insomnia （39.71％）,fatigue （30.88％） and heart palpitations （23.53％） were higher （P ＜ 0.05）.Conclusion： Hysterectomy may reduce the patient＇ s ovarian function,decrease its normal hormone levels,influence libido and life quality,and may increase the incidence of perimenopausal syndrome.
