胰腺损伤58例诊治体会 被引量：4

摘要 目的探讨胰腺损伤的诊断和治疗。方法回顾性分析1996年1月至2012年12月我院收治的58例胰腺损伤的临床资料。结果行清创外引流术17例;胰远端切除术15例(保留脾脏3例);胰管端端吻合术2例;胰近端裂口缝合加远端空肠Roux-Y吻合术13例;5例Ⅴ级患者先行损伤控制性手术(damage control operation,DCO),4例生命体征稳定后行确定性手术;2例Ⅴ级患者行胰腺修补、十二指肠肠腔内置管引流术;胰十二指肠切除术4例。发生胰瘘14例,十二指肠瘘3例,胰腺假性囊肿3例。治愈48例,死亡10例。结论胰腺损伤的诊治应早诊断、早探查。根据胰腺损伤分级选择合适的手术方式可以提高存活率和减少并发症。
