A new method for reconstruction following whipple operation: Ileal interposed cholecysto-jejunostomy

目的:探讨采用间置回肠胆囊空肠吻合法在胰十二指肠切除术消化道重建中的应用。方法:采用在胆囊和空肠间间置末段40 cm带血供的回肠作为胆肠重建方式,从而将胆汁和胰液最后在消化道内混合,观察该术式的安全性、可行性和术后效果。结果:3例患者手术均获得成功,术后无胰瘘并发症发生。结论:采用间置回肠胆囊空肠吻合法行胰十二指肠切除术消化道重建安全、可行,其在降低胰瘘发生率方面的效果有待大宗病例的临床观察。

Objective： To investigate the use of ileal interposed cholecysto-jejunostomy in digestive tract reconstruction of pancreaticoduodenectomy. Methods： In jejunal interposition between the gallbladder and the last paragraph of 40 cm vascularized ileum as intestinal reconstruction, bile and pancreatic juice mixed in the digestive tract at last. To observe the surgical safety, feasibility and postoperative results. Results： Three cases were successful surgery, with no postoperative pancreatic fistula. Conclusion： The method of ileal interposed cholecysto-jejunostomy in digestive tract reconstruction of pancreaticoduodenectomy is safe and feasible, its effect in reducing the incidence of pancreatic fistula need large series of cases to be confirmed.
