补肾活血汤治疗腰椎间盘突出引起疼痛的临床研究

Bushen Huoxue Decoction in Treatment of Pain due to Lumbar Disc Herniation

目的:探讨补肾活血汤加减对治疗腰椎间盘突出引起的疼痛的临床效果,并进行分析。方法:腰椎间盘突出疼痛严重患者86例,按数字法随机分为观察组和对照组,观察组46例,对照组38例。对照组采用常规治疗处理,观察组除基础治疗外,采用补肾活血汤加减口服治疗,并对两组患者进行回顾性对照研究。观察组治疗方案:在基础治疗上给予中药补肾活血汤口服,每天1剂,早晚分服;同时中药腰部雾化,每天1次,10 d为1疗程;对照组治疗方案:基础治疗包括腰椎推拿+静滴5%葡萄糖250 mL,香丹注射液20 mL;5%葡萄糖250 mL,川芎嗪240 mg;甘露醇250 mL,内加地塞米松5 mg静滴qd,7 d为1疗程;对两组的临床效果进行比较。结果:观察组与对照组的临床治愈率分别为93.48%(43/46)、81.58%(31/38)。两组临床治愈率具有显著性差异(χ2=3.773,P<0.05);观察组腰痛、下肢麻木、下肢放射痛、直腿抬高实验4项观察指标明显改善,与对照组相比较,改善程度优于对照组(t=3.918,P<0.05);观察组腰痛持续时间、下肢麻木减轻或消失时间、下肢放射痛时间明显短于对照组(t=2.098,P<0.05),且住院天数明显短于对照组(t=2.818,P<0.05)。结论:采用补肾活血汤加减治疗腰椎间盘突出引起的疼痛,能够达到腰痛症状缓解明显,缩短治疗时间,临床效果疗效显著,为临床值得应用的一种方法。

Objective：To investigate the clinical effect of Bushen Huoxue Decoction in the treatment of pain due to lumbar disc herniation caused pain. Methods ： 86 cases of hospitalized patients of severe pain due to lumbar disc hernia- tion, according to figures were randomly divided into observation group and control group ：46 cases in observation group, 38 cases in the control group. The control group used the conventional treatment, and the observation group in addition to the basic treatment, added Bushen Huoxue Decoction oral treatment, and two groups＇ patients were studied by the the ret- rospective case- control study. The observation group therapy：Bushen Huoxue Decoction orally given besides the basic treatment, one dose a day in the morning and evening ; at the same time traditional Chinese medicine atomization at the waist,once a day, 10 days for a course of treatment. The control group therapy ：basic treatment regimen including lumbar massage ＋ intravenous 5% glucose 250 mL,Xiangdan Injection 20 mL in 5% glucose 250 mL;240 mg ligustrazine;man- nitol 250 mL with 5 mg dexamethasone intravenously once a day,7 days for a course of treatment. The clinical effects of two groups were compared. Results ： The clinical cure rates of observation group and control group were 93.48% （43/46）, 81.58% （31/38）. The clinical cure rate of two groups had significant difference （χ^2 = 3. 773 ,P 〈 0.05 ）. The observa- tion group＇ s four observation indexes include low back pain, radiating pain of lower limb, numbness and straight leg rai- sing test were improved significantly compared with those of the control group and the degree of improvement was better than that of control group （ t = 3. 918 ,P 〈 0.05 ）. The observation group＇ s low back pain and lower limb numbness re- duce or duration as well as the disappearance time of radiating pain of lower limb was shorter than those of the control group （ t = 2. 098, P 〈 0.05 ） , and the length of hospital stay was shorter than that of the control group （ t = 2.818, P 〈0. 05 ）. Conclusion：The use of Bushen Huoxue Decoction in the treatment of pain caused by lumbar disc herniation can re- lieve the pain significantly and shorten time of therapy with obvious clinical curative effect and it is a method for clinical application.
