大青龙汤联合西药治疗败血症的临床研究

Clinical Research of Traditional Chinese Medicine Combined with Western Medicine in Treatment of Sepsis

目的:比较大青龙汤联合西药与西药单用治疗败血症的应用效果,以探讨大青龙汤辅助治疗败血症的临床优势。方法:回顾性分析医院收治的成年败血症患者72例,其中36例采用抗生素对症疗法(对照组),36例在此基础上加用大青龙汤(观察组)。记录临床疗效、机体评分及血象变化,同时,取患者空腹外周血,以流式细胞仪检测CD+14CD+16细胞计数,以ELISA试剂盒检测TNF-α和IL-10水平。结果:观察组患者的有效率显著高于对照组,死亡率显著低于对照组(P<0.05)。治疗前,两组患者之间的各项观察指标均无统计学差异(P>0.05)。治疗后,观察组APACHⅡ、SIRS评分明显低于对照组;外周血白细胞、中性粒细胞及CD+14CD+16细胞计数明显低于对照组;同时,TNF-α水平偏低,IL-10水平偏高;且各项指标之间的差异性均有统计学意义(P<0.05)。结论:大青龙汤辅助西药治疗败血症,可更好的发挥免疫调节作用,加强抗炎和抗感染效能,利于提高临床疗效。

Objective ： To explore the advantage of traditional Chinese medicine in the treatment of sepsis by contrastive analysis. Methods ：72 cases of patients with sepsis were selected. 36 cases treated with antibiotics only were control group and 36 cases treated with Chinese medicine Daqinglong Decoction were observation group. Clinical curative effect, physi- ology score and blood picture indexes were recorded. At the same time,CD14＋ CD16＋ cells＇count of peripheral blood was de- tected with flow cytometry and serum TNF - α and IL - 10 contents were detected with ELISA kits. Results ： The effective rate of observation group was significantly higher while the mortality rate was significantly lower than that of control group （ P 〈 0.05 ）. There was no significant difference between groups before treatment in the indexes we studied. After treat- ment, the APACH 11 and SIRS score of observation group were lower and peripheral blood leukocytes, neutrophils andCD14＋ CD16＋ ceils＇ counts were also lower than those of control group （P 〈 0.05 ）. Furthermore, the serum TNF - α con- tent of observation group was lower while IL - 10 was significantly higher than that of control group （P 〈 0. 05 ）. Conclu- sion ： Our research shows that Daqinglong Decoction has advantage on improving the immune function of patients with sep- sis, which is benefit for anti - inflammation and anti - infection.
