血浆内脂素水平与心肌梗死发病风险的Meta分析 被引量:2

摘要 目的 探讨血浆内脂素水平与MI发病风险的具体关联.方法 计算机检索PubMed、CBM、万方、维普、CNKI等中英文数据库,检索范围从建库至2013年7月1日,采用STATA 12.0软件进行Meta分析,计算标准均数差(SMD)及其95%可信区间(95%CI),评价两者关联性.结果 本Meta分析共计纳入了11项病例对照研究,包括362例MI患者和322名健康对照人群.Meta分析结果表明,血浆内脂素水平与MI发病风险增加有关(SMD=3.82,95%CI 2.67~4.98,P<0.01).根据对照组来源不同进行亚组分析发现,无论是普通人群还是来医院体检人群,血浆内脂素水平与普通人群和医院人群的MI发生风险增加均有关(普通人群SMD=4.12,95%CI 2.23~6.01,P<0.01;医院人群SMD=3.65,95%CI 2.67~4.98,P<0.01).结论 血浆内脂素水平可能与MI发病风险增加有关,它可能是MI早期诊断和预后判定的重要分子标志物. Objective Emerging evidences have shown that elevated plasma level of visfatin may con- tribute to the risk of myocardial )infarction (MI), but individually published studies showed inconclusive results.This study aimed to derive a more precise estimation of the association between plasma visfatin level and MI risk through a detailed meta-analysis of studies published in peer-reviewed journals. Methods A literature search of PubMed, China BioMedicine (CBM), WANFANG, VIP, China National Knowledge Infrastructure (CNKI) databases was conducted on articles published before July 1st, 2013. Meta-analysis was conducted with the use of the STATA 12.0 software. Crude standardized mean difference(SMD) with 95% confidence interval(C/) was cal- culated. Eleven case-control studies were included with total 362 MI patients and 322 healthy controls. Results The meta-anaiysis results revealed that elevated plasma level of visfatin was associated with increased risk of MI (SMD=3.82, 95%CI 2.67-4.98, P〈0.01 ). Further stratification analysis by source of controls, the results showed that elevated plasma level of visfatin showed significant associations with increased risk of MI in both hospital- based and population-based studies (SMD=4.12, 95%CI 2.23-6.01, P〈0.01, SMD=3.65, 95%CI 2.67-4.98, P〈0.01; respectively). Conclusion The current meta-analysis indicates that elevated plasma level of visfatin may contribute to the risk of MI. Plasma visfatin level may be a promising biomarker for detection and prognosis of MI.
