摘要
目的探讨超声造影中的时间-强度曲线在眼内肿瘤中的诊断价值。方法选取2011年9月至2013年5月间收治的单眼发病的眼内肿瘤患者90例,进行常规超声、彩色多普勒超声和超声造影的观察和诊断。结果患者常规超声影像学特征主要为形状不规则,边界不清,内部回声强,部分钙化。在血流参数中,患眼的收缩期峰值流速(PSV)、舒张末期流速(EDV)和阻力指数(RI)值显著高于健眼,差异有统计学意义(P<0.05)。在造影参数比较中,患眼的峰值强度、曲线尖度和曲线下面积显著高于健眼,而达峰时间显著低于健眼,差异有统计学意义(P<0.05)。采用超声造影技术共诊断出85例眼内肿瘤患者,总检出率为94.4%。其中泪器肿瘤的检出率为100.0%,血管瘤误诊3例,淋巴瘤误诊2例。结论超声广泛应用于眼内肿瘤的诊断中,而超声造影中的时间-强度曲线可准确反映眼内肿瘤的微血管增强模式,增加诊断的准确性。
Objective To evaluate the diagnostic value analysis of time - intensity curve of ultra- sound contrast for intraoeular tumors. Methods A total of 90 cases patients with monocular intraocular tumors were selected. All patients were diagnosed and treated with conventional ultrasound, color Doppler ultrasound and ultrasound contrast. Results The imaging characteristics of conventional ultrasound were ir- regular shape, unclear boundary, strong internal echo and partial calcification. The values of PSV, EDV and RI for the ill eyes were significantly higher than that for the healthy one in the flow parameters. The difference was statistically significant ( P 〈 0.05 ). The peak intensity, sharp curve degree and area under the curve of the ill eyes were significantly higher than that for the healthy one in the contrast indicators, however, the time of maximum peak was significantly less than that for the healthy eyes ( P 〈 0. 05 ). 85 ca- ses of patients were detected using ultrasound contrast, and the total detection rate was 94.4%. In addition, the detection rate of lacrimal tumor was 100. 0%. Conclusion The application of ultrasound diagnosis is widespread in intraocular tumors, the time - intensity curve of angiography can accurately reflect tumor mi- erovaseular enhanced mode and then further increase the accuracy of diagnosis.
