心脏不停跳行重症二尖瓣置换术46例

目的 :介绍在体外循环心脏不停跳行重症二尖瓣置换术的经验。方法 :在不阻断主动脉 ,不灌注停跳液 ,心脏跳动下行重症二尖瓣置换术 46例 ,心胸比 0 .70～ 0 .88,平均 0 .77,心脏 B超 L VED70～ 89mm ,平均 79m m,EF0 .36～ 0 .6 3,平均0 .5 1,体外循环时间 2 8～ 6 8m in,平均 46 min。结果 :全组术后无低心排综合征及严重心律失常和胸部并发症。结论 :这种方法能更有效地保护心肌功能 ,简化操作 ,缩短体外循环时间和手术时间 。

Objestive: To review the exprience of severe mitral valve replacement under the condition of cardiopulmonary bypass with heart beating Method:46 patients of severe mitral valve replacement were performed,without aortic crossclamping and cardiophegin and with heart beating The cardiac thoracic ratioes of the cases were 0 70~o 88,averege0 77 Echocardingraphy showed that the LVED were 70~89mm,average 79mm,EF0 36~0 63,average 0 51 The cardiopulmonary bypass time were 28 to 68 min; average 46min Result: None of the patients had low cardiac output syndrome, severe arrythmia and cerebral complications Conclusion: The study results suggest that this method appears to be less performed, cardiopulmonary bypass time and operation time, better preserved heart-function The patients could recover more quickly
