慢性心力衰竭患者血浆脑钠肽水平改变与左心室功能的相关性分析

目的:探讨脑钠肽与慢性心力衰竭患者左心室功能的相关性。方法:所选的200例慢性心力衰竭患者根据心功能评定标准进行心功能分级,分为Ⅱ级组、Ⅲ级组、Ⅳ级组。同时选择同期到我院体检的健康者60例,作为对照组。测定所选对象的脑钠肽水平。结果:Ⅱ级组、Ⅲ级组、Ⅳ级组的脑钠肽水均高于对照组,差异有统计学意义(P<0.05);Ⅱ级组、Ⅲ级组、Ⅳ级组的左室射血分数均低于对照组,差异有统计学意义(P<0.05);Ⅳ级组脑钠肽水高于Ⅲ级组组、Ⅳ级组左心室射血分数低于Ⅲ级组,差异有统计学意义(P<0.05);Ⅲ级组脑钠肽水高于Ⅱ级组、Ⅲ级组左心室射血分数低于Ⅱ级组,差异有统计学意义(P<0.05)。结论:血浆脑钠肽水平改变有助于了解心力衰竭患者心功能状态,有助于评估患者预后,具有重要临床意义。

Objective To explore the relationship between the changes of plasma brain natriuretic peptide level in patients with chronic heart failure and left ventricular function. Methods 200 cases with heart failure were selected, and they were divided into grade II group, grade III group, grade IV group according to the cardiac function evaluation standard. The 60 healthy persons in same period were selected as control group, plasma brain natriuretic pcptide level were measured . Results The level of plasma brain natriuretic peptide level in grade II group, grade III group, grade IV group were higher than those in the control group, the difference was statis- tically significant （P 〈 O. 05 ） ; Left ventricular ejection fraction in grade II, III, IV group were lower than that in the control group, the difference was statistically significant （ P 〈 0. 05 ） ; The level of plasma brain natriuretic peptide in grade IV group were higher than those in grade III group, Left ventricular ejection fraction in grade IV group were lower than that in the grade III group, the difference was statistically significant （ P 〈 0.05 ） ; The level plasma brain natriuretic peptide in grade III group were higher than those in grade II group, Left ventricular ejection fraction in grade III group were lower than that in the grade II group, the difference was statistically significant （ P 〈 0. 05 ）. Conclusion The level change of plasma brain natriuretic peptide helps to understand the left ventricular func- tion and evaluate the prognosis of patients, which is great significance.
