瑞芬太尼用于患者清醒状态下无痛人工流产的临床观察 被引量:6

摘要 目的 观察瑞芬太尼用于无痛人工流产的可行性与安全性,并与使用丙泊酚进行比较.方法 选择门诊ASA Ⅰ~Ⅱ级早孕自愿选择人工流产术者120例,采用数字表法随机分成两组,每组60例,Ⅰ组给予丙泊酚2.5 mg/kg静脉注射,时间>30 s,体动明显时追加丙泊酚1.0 mg/kg;Ⅱ组给予瑞芬太尼0.5 μg/kg静脉注射,之后每间隔1分钟给予相同剂量瑞芬太尼致吸宫操作完毕停止给药.监测HR、SpO2、RR、SBP等各项生理参数.记录术毕清醒时间及离院时间,并观察两组术中镇痛效果.结果 两组HR、SBP、RR、SpO2术前(t=0.847、0.417、0.614、0.203)、术后(t=1.381、1.502、0.572、0.131)差异均无统计学意义(均P >0.05);两组间比较,术中HR、SBP、RR、SpO2 Ⅰ组明显低于Ⅱ组(t=4.731、5.230、2.673、2.659,均P<0.05);两组术前与术中HR、SBP、RR、SpO2比较,Ⅰ组术中低于术前(t=9.605、10.306、5.263、4.917,均P<0.05),Ⅱ组术前与术中差异无统计学意义(t=1.603、1.597、1.257、0.739,均P>0.05).Ⅱ组术中、术毕始终保持意识清醒,离院时间Ⅱ组短于Ⅰ组(t=2.632,P<0.05).两组术中镇痛优良率均为100%.结论 瑞芬太尼应用于无痛人工流产术,使患者镇痛、镇静充分,术中意识清醒,并发症少,术后即可离院,值得临床推广应用.
