摘要
目的研究慢性肾脏病(CKD)患者血尿酸水平与颈动脉硬化的关系。方法将122例CKD患者按不同分期分为I~II期、III~Ⅳ期、V期3组,并选取同期体检的40例非CKD患者作为对照组,分别采用酶比色法测定血尿酸(SUA)浓度,并采用彩超行双侧颈动脉检查,观察颈动脉内膜一中层厚度(IMT)及粥样硬化斑块。结果CKD组SUA浓度、HUA检出率、动脉硬化检出率均高于对照组(P〈0.05或P〈0.01)。I~II期、III~Ⅳ期、V期组SUA浓度逐渐升高,HUA检出率也逐渐增大,组间比较差异有统计学意义(P〈0.05);Spearman相关性分析显示,SUA浓度与肾小球滤过率(GFR)呈负相关(P〈0.01)。V期组颈动脉硬化检出率高于I~II期组和III~Ⅳ期组(KO.05);HUA组颈动脉硬化检出率高于非HUA组(P〈0.05);SUA浓度与颈动脉硬化发生率呈正相关(P〈0.05)。结论CKD患者SUA浓度升高,易发生HUA,而颈动脉硬化程度与SUA呈正相关,提示CKD合并HUA患者易发生颈动脉硬化。
Objective To study the correlation of serum uric acid in patients with chronic kidney disease with carotid atherosclerosis.Methods 122 patients with different stages of CKD patients were divided into stage I -II, III-Ivstage, V of three groups,and select the corresponding period in 40 patients with non-medical CKD patients as a control group,respectively,using enzymatic colorimetrie determination of serum uric acid (SUA) concentration, and the use of bilateral carotid ultrasound examination,observation carotid intima-media thickness (IMT) and atherosclerotic plaques. Results CKD group SUA concentration,HUA detection rate,arteriosclerosis detection rate higher (P〈0.05 or P〈0.01). Stage I -If, III-Ivstage, V SUA concentration of group gradually increased, HUA also increases the detection rate between the two groups was statistically significant (P〈0.05) difference; Spearman correlation analysis, SUA concentration and glomerular filtration rate (GFR) was negatively correlated (P〈0.01). Vdetection rate of carotid atherosclerosis in stage I-Iigroup andIII-Ivstage group (P〈0.05); HUA detection rate of carotid atherosclerosis in non-HUA group (P〈0.05); SUA concentration The incidence of carotid atherosclerosis was positively correlated (P〈0.05). Conclusion CKD patients with elevated concentrations of SUA, prone HUA,and the degree of carotid atherosclerosis was positively correlated with SUA, suggesting that CKD patients merge HUA prone to carotid atherosclerosis.
