摘要 目的探讨肾上腺节细胞神经瘤的CT表现。方法回顾性分析经病理证实的14例肾上腺节细胞神经瘤CT平扫及增强表现特点。结果14例病灶大小平均为(41.3±28.5)mm(23.8~74.3mm),境界清楚,呈椭圆形(7/14)、水滴形(3/14)、不规则形(4/14);瘤内钙化(3/14),包膜钙化(1/14),压迫血管但无狭窄(4/14)。肿瘤平扫为均匀的低密度[(28.3±9.8)HU],增强扫描主要有3种类型:(1)动、静脉期均为均匀轻度强化,强化幅度均〈20HU(5/14)。(2)动、静脉期为不均匀轻度强化,不均匀区呈曲线状、片絮状中度强化,强化幅度20~50HU(8/14)。第(1)、(2)两种类型的轻度强化区(增值〈20HU)占瘤体总容积80%以上。(3)动、静脉期呈中度以上渐进性强化(1/14)。结论肿瘤呈较低密度[(28.3±9.8)HU]表现,轻度强化区在瘤体总容积的高占比(〉80%)是肾上腺节细胞神经瘤的CT特征表现。 Objective To explore the imaging findings of adrenal ganglioneuroma. Methods CT findings on both plain and dynamic enhanced CT scanning of 14 patients with pathologically-proved adrenal ganglioneuromas were retrospec- tively analyzed. Results The diameter of tumors was (41.3 ± 28.50 ) ram, ranged from 23.8 to 74.3 mm. All tumors were well defined. The lesion' s shape was oval ( n = 7), water drop ( n = 3 ) or irregular ( n = 4). Other findings included calcification within the tumor (n = 3 ), capsule calcification (n = 1 ) and vessel compression with no stenosis (n =4). The lesion was hypo-density (28.3 ±9.8 HU) on plain scans. On enhanced CT scans the lesions could be divided into three types. (1) The lesions showed slight homogeneous enhancement ( 〈 20 HU) at both arterial and venous phases (n = 5). (2) The lesions showed patch slight heterogeneous enhancement (20 -50 HU) at both arterial and venous phases (n = 5 ). ( 3 ) The lesion showed moderate enhancement, which was increasing with time, in both arterial and venous phases ( n = 1 ). Conclusion The sign of low-density and the high pereentage of relatively-low enhancement area ( 〉 80% ) are the characteristic CT features of adrenal ganglioneuroma.
