ADC值、FA值与颈椎间盘退变程度的相关性研究 被引量:8

Correlation Study on ADC and FA Values with the Degeneration Degree of Cervical Intervertebral Discs

摘要 目的探讨磁共振扩散张量成像(DTI)表观扩散系数(ADC)、各向异性分数(FA)定量评估颈椎间盘退变程度的价值。方法随机选取61例颈肩部、上肢不适者行常规MRI、矢状位DTI。测量各颈椎间盘髓核ADC值、FA值,行Miyazaki颈椎间盘退变评级,观察椎间盘形态,分析不同退变分级、不同形态椎间盘与ADC值、FA值的相关性,比较ADC值与FA值的诊断价值。结果 ADC值、FA值与Miyazaki分级具有相关性(r=-0.557,P<0.05;r=0.498,P<0.05);突出比无膨出/突出椎间盘的ADC值低,FA值高(P<0.05);ADC值的敏感性、特异性比FA值高。结论 ADC值、FA值能够定量评估颈椎间盘的退变程度,且ADC值比FA值诊断价值高。 Objective To discuss the value of the apparent diffusion coefficient (ADC) and fractional anisotropy (FA) of diffusion tensor imaging (DTI) in quantitatively assessing the degeneration degree of cervical intervertebral discs. Methods A total of 61 patients with ache or numbness at neck, shoulder and upper limb were randomly selected for the study. Routine MRI and sagittal DTI scanning were performed in all patients. ADC and FA values of nucleus pulposus of each cervical intervertebral disc were measured. The discs were classified according to the Miyazaki grading and the mor- phology of each intervertebral disc was determined. The correlations of different degeneration grade and disc morphology with ADC and FA values were analyzed. The diagnostic value of ADC was compared with FA. Results Both ADC values and FA values were closely correlated with the Miyazaki grades ( r = - 0. 557, P 〈 0. 05 ; r = 0. 498, P 〈 0.05 ). ADC val- ues of protruded discs were lower than that of non-bulged or non-protruded discs, while FA values of protruded discs were higher than that of non-bulged or non-protruded discs (P 〈 0. 05). ADC value was superior to FA value in its higher sensi- tivity and specificity. Conclusion ADC and FA values can be effectively used to quantitatively assess the degeneration degree of cervical intervertebral discs, and the diagnostic value of ADC is higher than that of FA.
