摘要 目的动态观察急性脑梗死患者不同时期外周血单个核细胞(PBMC)中CD4+CD25+CD127low调节性T细胞(Treg)表达,探讨其对急性缺血性脑卒中免疫状态及病程进展的影响。方法选取急性脑梗死患者60例为脑卒中组,并分为发病24~48 h组(n=16)、3~7 d组(n=22)和8~14 d组(n=22);另选取健康体检者22例为对照组。采用流式细胞术检测60例急性脑梗死患者和22例健康体检者外周血中CD4+CD25+CD127lowTreg水平并进行分析。结果与对照组比较,脑卒中组外周血CD4+CD25+CD127low、CD4+CD25highTreg比例在起病后24~48 h、3~7 d时显著降低(P<0.01);8~14 d逐渐恢复至正常水平,与对照组比较差异无统计学意义(P>0.05);与对照组比较,脑卒中组的CD4+T细胞比例升高,差异有统计学意义(P<0.05)。结论 Treg可能参与急性缺血性脑卒中的病理生理过程,其功能失衡在急性缺血性脑卒中炎性损伤中具有重要作用,早期检测可能为其治疗提供依据。 Objective To investigate the expression of CD4 +CD25 +CD127 low regulatory T cells in peripheral blood mononuclear cells( PBMNCs) in order to explore the influence on the immune status and disease progression in the different period of acute ischemic stroke. Methods 60 patients with acute cerebral infarction were selected as the stroke group,and divided them into the 24-48 h group (n=16), 3-7 d group (n=22) and 8-14 d group (n=22);22 healthy human were set as the control group. To analyze the percentage of CD4 +CD25 +CD127low regu-latory T cells in the peripheral blood of acute cerebral infarction patients and healthy human with flow cytometry. Results The percentages of CD4 +CD25 +CD127 low and CD4 +CD25 high regulatory T cells in the peripheral blood of the stroke group were significantly decreased at 24 -48 h,3 -7 d ( P 〈0.01 ) when compared with the control group, and then tended to return toward the control value at 8-14 d (P〉0.05);the percentage of CD4 + regula-tory T cells in the peripheral blood of the stroke group was significantly higher than control group ( P 〈0.05 ) . Conclusion Imbalance of regulatory T cells is very likely to play an important role in the immunological injury of acute ischemic stroke. Regulatory T cells may be involved in the pathogenesis of acute ischemic stroke,and early detection may provide a basis for treatment.
