桑蒺合剂干预高血压病(肝火亢盛夹痰证)患者免疫与炎性反应的临床研究

Clinical research of Sangji Mixture on the immunological and inflammatory reaction in hypertension of overabundant liver-fire with phlegm syndrome

目的:观察桑蒺合剂对高血压病(肝火亢盛夹痰证)患者Toll样受体4(toll like recepter 4,TLR4)及其下游炎性因子白介素-6(interleukin 6,IL-6)、肿瘤坏死因子(tumor necrosis factor-α,TNF-α)的影响。方法:60例高血压肝火亢盛夹痰证患者,随机分为治疗组和对照组各30例,均予非洛地平和(或)贝那普利治疗28d,治疗组加用桑蒺合剂,检测治疗前后血压、中医证候积分、外周血单核细胞表面TLR4及血清IL-6、TNF-α等指标。结果:治疗后两组患者血压、中医证候积分、TLR4、IL-6、TNF-α较治疗前降低(P<0.05),且治疗组除收缩压外均较对照组下降更明显(P<0.05)。结论:桑蒺合剂可改善肝火亢盛夹痰证高血压病患者的临床症状,减少外周血单核细胞表面TLR4及炎性因子IL-6、TNF-α的表达,在一定程度上抑制免疫与炎性反应。

Objective： To observe the effect of Sangji Mixture in the treatment of the hypertention patients overabundant liver-fire with phlegm syndrome） via the toll like recepter4 （TLR4）, inflammatory factors including interleukin 6 （IL-6） and tumor necrosis factor-α（TNF-α）. Methods： 60 cases of hypertension with overabundant liver-fire with phlegm syndrome, were randomly divided into treatment group and control group with 30 cases in each, both received Benazapril and （or） Felodipine treatment, the treatment group received Sangji Mixture as additional treatment. Throughout the 28 days observation period, the indicators such as blood pressure, symptom scores, peripheral blood mononuclear cells TLR4, IL-6 and TNF-α were measured before and after treatment. Results： After the treatment, the blood pressure, symptom scores, peripheral blood mononuclear cells TLR4, IL-6 and TNF-α of the two groups were both decreased compared with the data before the treatment （P〈0.05）, while the treatment group decreased more significantly than the control group （P〈0.05）. Conclusion： Sangji Mixture can improve the anti-hypertensive efficacy, alleviate the clinical symptoms of patients who are diagnosed as ＂overabundant liver-fire with phlegm syndrome and decrease the expression of Toll Like Recepter4 （TLR4）, inflammatory factors IL-6 and TNF-α located on the peripheral blood mononuclear cell surface, consequently suppresses the immunological and inflammatory reaction.
