腹腔镜与开腹手术治疗子宫肌瘤疗效对比

Ccomparative study of curing fibroid by laparoscopic operation and open operation

目的对比分析腹腔镜与开腹手术治疗子宫肌瘤的临床疗效。方法选取2011年1月～2013年6月在我院妇产科就诊的子宫肌瘤切除术患者116例，根据患者手术方法不同将其分为观察者与对照组，每组各58例，观察组进行腹腔镜切除术治疗，对照组患者给予常规开腹切除术。观察术后疗效及术后并发症发生情况并进行对比分析。结果观察组手术时间（103.3±20．8）min、术中出血量（100．4±50．6）mL、肛门排气时间（20．6±9．7）h、术后下床活动时间（1-3±0．6）h、抗生素应用时间（5．5±1．5）d、术后住院时间（7．8±2．1）d，明显优于对照组手术时间（126．5±30．6）min、术中出血量（145．8±60．7）mL、肛门排气时间（30-3±10．8）h、术后下床活动时间（2．2±1．2）h、抗生素应用时间（7．5±2．5）d、术后住院时间（16．2±3．1）d，差异有统计学意义（P〈0．05）。两组患者术后并发症发生率比较，观察组1．72％明显低于对照组12．07％，差异有统计学意义（Х^2=5．94，P〈0．05）。结论腹腔镜手术治疗子宫肌瘤疗效较好，术后恢复快，安全性高，值得临床推广应用。

Objective To compare the clinical effect of curing fibroid by laparoscopic operation and open operation. Methods The clinical data of 116 fibroid patients who were in hospital from 2011 January to 2013 June were retrospectively analyzed. All patients were divided into the control group and the experiment group according to the treatment method, each group 58 cases. The experiment group was given laparoscopic operation and the control group was treated by open operation. The operation effect and postoperative complications of the two groups were compared. Results In experiment group, the operation time was （103.3 ± 20.8）min, the intraoperative blood loss was （100.4 ± 50.6）mt, anal exhaust time was （20.6 ± 9.7）h, postoperative ambulation time was （1.3 ± 0.6） h, antibiotic application time was （5.5 ± 1.5）d, and postoperative hospitalization time was （7.8 ± 2.1）d, which significantly better than the control group operation time （126.5 ± 30.6min）, amount of bleeding（145.8 ± 60.7）M1, anal exhaust time（30.3 ± 10.Sh）, postoperative ambulation time（2.2 ± 1.2h）, antibiotic application time（7.5 ± 2.5d）, postoperative hospitalization time （16.2 ± 3.1d）. The difference was statistically significant （P 〈 0.05）. The incidence of postoperative complications 1.72% of the experiment group was lower than that of the control group 12.07%, the difference was statistically significant（Х^2=5.94, P 〈 0.05）. Conclusion Laparoscopic operation in the treatment of uterine fibroids is effective, faster postoperative recovery, and high security, It is worthy of clinical application.
