Problems and solutions on dual referral

目的:采用循证医学的方法探讨我国目前医疗体制下双向转诊存在的问题及解决对策。方法:系统检测医学相关数据库Medline、中国知网(CNKI)、万方数据库、维普数据库(VIP)和中国生物医学文摘数据库(CBM),收集关于我国双向转诊相关文献,采用系统评价的方法对我国目前双向转诊中的问题及解决对策进行分析探讨。结果:根据文献纳入排除标准,最终有56篇相关文献纳入分析。研究结果显示,目前我国在实施双向转诊的过程中也存在很多问题,如双向转诊认可度及居民社区就诊率较低,转诊单位之间存在经济壁垒等,均给双向转诊带来一定的阻碍。结论:我国部分城市已经制定了一些双向转诊的实施方案,相关的试点工作已取得了一些成绩,但双向转诊工作作为我国医疗体制改革的重点工作之一仍需不断完善和改进。

Objectives： To evaluate the problem and solution in the process of dual referral under current medical management system by evidence based medicine. Method： Papers about the dual referral were searched in the medicine associated databases such as Medline, CNKI, Wanfang, VIP and CBM. The problem and solution methods of dual referral were evaluated by systematic review method. Results： 56 studies were finally included in this analysis with the included and concluded standard. The analysis demonstrated that our country in the process of implementation of dual referral also exist many problems, such as two-way referral recognition and neighborhood administration low, economic barriers between referral units, etc., All of them bring dual referral certain obstacles. Conclusions： Some cities in China have developed dual referral implementation scheme. The relevant pilot work has made some achievements, but the dual referral work as one of the key work of medical system reform in our country still need to constantly improve.
