小剂量右美托咪定复合丙泊酚在无痛人流中的应用 被引量:8

摘要 目的探讨小剂量右美托咪定复合丙泊酚用于无痛人流的临床疗效。方法选取2012年12月至2013年3月本院收治的100例无痛人流术患者,随机分为P组(静脉输注0.9%生理盐水后给予丙泊酚2mg/kg)和D组(静脉输注右美托咪定0.5ug/kg后给予丙泊酚2mg/kg)。记录患者麻醉前(T0)、用右美托咪定或安慰剂后(T1)、手术开始时(T2)、扩宫颈时(T3)、手术结束时(T4)、苏醒时(T5)的平均动脉压(MAP)、HR、脉搏血压饱和度(SPO2),记录丙泊酚的初始用量、总用量、手术时间、清醒时间以及术中体动反应、注射痛、呼吸抑制、术后腹痛的发生情况。结果与T0比较,D组的MAP在T2、T3、T4时,HR在T1、T2、T3、T4时比较差异有统计学意义(P<0.05);与T2比较,P组的HR在T3时比较差异有统计学意义(P<0.05);与P组比较,D组MAP在T2、T3时,HR在T1、T2、T3、T4时比较差异有统计学意义(P<0.05);与P组比较,D组的丙泊酚总量、体动发生率、注射痛、术后腹痛发生率与P组比较差异有统计学意义(P<0.05)。两组的手术时间、丙泊酚初始用量、清醒时间、SPO2比较差异无统计学意义(P>0.05)。结论 0.5μg/kg小剂量右美托咪定复合丙泊酚用于无痛人流术中,丙泊酚用量小,血流动力学稳定,体动反应少,且注射痛、术后疼痛等不良反应减低。
