摘要
目的了解老年人血糖异常检出及与体质指数的相关性。方法 6830名老年人根据空腹血糖值分为空腹血糖正常(NGT)组、空腹血糖受损(IFG)组和糖尿病(DM)组,分别记录性别、年龄,测量收缩压(SBP)、舒张压(DBP)、腰围(WC)、臀围、身高、体质量,计算体质指数(BMI)、腰臀比(WHR),检测空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C),进行组间各指标的比较及空腹血糖受损情况与其它因素相关性分析,为糖尿病高危人群的筛检提供依据。结果 NGT 3200例(46.85%),IFG 2575例(37.70%),DM 1055例(15.45%);随着BMI的增加,血糖异常的发生率逐渐升高,IFG与DM组中超重与肥胖人群所占比例最高;按NGT、IFG、DM顺序,BMI、WHR、WC、TG、HLD-C与SPD平均水平依次升高,HLD-C平均水平依次降低,三组间差异均有统计学意义(P<0.05);IFG组与NGT组LDL-C、DBP和TC比较差异有统计学意义(P<0.05);IFG与DM组的LDL-C和DBP比较差异有统计学意义(P<0.05);空腹血糖受损与BMI、WHR、WC、SBP、LDL-C、TG成正相关,与HDL-C负相关;空腹血糖受损多因素logistic回归分析发现BMI、WHR、WC、SBP、LDL-C、TC和年龄对空腹血糖受损有较大影响。结论老年人血糖异常率随着BMI的增加发生率逐渐升高。BMI、WHR、WC及多种代谢指标与空腹血糖受损成线性相关,对空腹血糖受损有较大影响。
Objective To observe the relationship between abnormal glucose and body mass index(BMI)in seniorcitizens. Methods A total of 6830 senior citizens from a community of Hangzhou were selected to measure fastingglucose and then were accordingly divided into normal glucose group(NGT), injured fasting glucose(IFG)group anddiabetes group. The gender, age, systolic blood pressure(SBP), diastolic blood pressure(DBP), waist circumference(WC), hip circumference (HC), height, and body weight of each person were recorded and then the BMI andweight to hip ratio(WHR)were calculated. The levels of triglyceride(TG), total cholesterol(TC), high-densitylipoprotein-cholesterol(HDL-C), and low-density lipoprotein-cholesterol(LDL-C)were determined and compared a-mong the three groups. The risk factors for abnormal glucose were analyzed. Results There were 3200 citizens(46.85%)in NGT group, 2575(37.70%)in IFG group, and 1055(15.45%)in diabetes group. The incidence ofabnormal glucose was increased as BMI increased; IFG and diabetes groups had more overweight and obese per-sons. The BMI, WHR, WC, TG, HLD-C, and SBP increased in NGT, IFG, and diabetes groups in order, all withsignificant differences among the three groups(all P〈0.05). Significant differences in LDL-C, DBP, and TC werenoted between IFG group and NGT group(P〈0.05). The levels of LDL-C and DBP were statistically different between IFG and diabetes groups(P〈0.05). Injured fasting glucose was correlated with BMI, WHR, WC, SBP, LDL-C,and TG and was negatively related to HDL-C. Logistic regression analysis showed that injured fasting glucose wasaffected by BMI, WHR, WC, SBP, LDL-C, TC and age. Conclusion The incidence of abnormal glucose increasedas the BMI increased in senior citizens. BMI, WHR, WC and other metabolic indexes were correlated with injuredfasting glucose.
