无创辅助通气联合纳洛酮治疗COPD合并呼吸衰竭疗效

Curative effect of NIPPV combined with naloxone in the treatment of COPD patients complicated with respiratory failure

目的探讨无创双水平正压通气(BiPAP)联合纳洛酮(NLX)治疗COPD并发呼吸衰竭的治疗效果。方法将2010年2月到2011年12月住院治疗的96例COPD并发呼吸衰竭患者随机为对照组与观察组,两组患者均给予无创双水平正压通气治疗,观察组同时应用纳洛酮(NLX)。治疗48 h后比较两组的疗效、死亡率、治疗前后呼吸次数、心率、以及血气分析指标的变化。结果观察组治疗有效率高于对照组(85.41%VS 66.67%,P<0.05);观察组与对照组住院期间死亡率无显著差别;治疗后48 h观察组呼吸次数和心率下降,血气分析指标PH及PaO2升高,PaCO2下降,效果优于对照组,差别有统计学意义(P<0.05)。结论与对照组相比,观察组应用无创呼吸机联合纳洛酮治疗COPD并发呼吸衰竭可显著改善血气分析指标,提高临床疗效。

Objective To investigate the curative effect of noninvasive BiPAP combined with naloxone in the＆amp;nbsp;treatment of elderly COPD patients complicated with respiratory failure. Methods 96 elderly COPD patients compli-cated with respiratory failure were randomly divided into two groups：the control group and the observation group. All patients were treated with noninvasive mechanical ventilation, while the observation group was additionally treated with naloxone. The efficacy of the two groups was compared before and after the treatment through mortality, respira-tory rate, heart rate and blood gas analysis. Results The effective rate was obviously high in the observation group than in the control group （85. 41% VS 66. 67%, P〈0. 05）. There was no significant difference in mortality be-tween the two groups during hospital stay （P〉0. 05）. The improvement of respiratory rate, heart rate and blood gas analysis was more pronounced in the observation group than in the control group （ P〈0. 05 ） . Conclusion Com-pared with the control group, the application of noninvasive ventilation combined with naloxone can significantly im-prove the clinical efficacy and blood gas analysis index in the treatment of COPD patients complicated with respiratory failure.
