外剥内扎术与PPH治疗重度痔疮临床效果对比

Clinical effect comparison of Milligan-Morgan operation and procedure for proplapse and hemorrhoids in the treatment of severe hemorrhoids

目的对比外剥内扎术与吻合器痔上粘膜环形切除术(PPH)治疗重度痔疮患者的临床效果。方法选取重度痔疮患者150例,随机分为两组,各75例。对照组患者采用外剥内扎术治疗,观察组患者采用PPH治疗,观察及评价两组治疗效果。结果观察组患者手术时间、疼痛时间、创面愈合时间、住院时间及至恢复工作时间均短于对照组(P<0.01)。术后观察组肛缘水肿、尿潴留、便中带血、切口渗血等并发症发生率均低于对照组(P<0.05)。结论 PPH治疗重度痔疮临床效果优于外剥内扎术,建议在临床治疗中推广应用。

Objective To compare the clinical effect of Milligan-Morgan operation and procedure for proplapse and hemor-rhoids（PPH） in the treatment of severe hemorrhoids .Methods One hundred and fifty patients with severe hemorrhoids were analyzed and divided into two groups （ n =75 ） .The control group patients received Milligan-Morgan operation treatment .The treatment group pa-tients received PPH treatment .Clinical effect was observed after treatment .Results The operative time ,pain time,wound healing time , hospitalization time and return-to-work time of treatment group were significantly shorter than that of control group （ P 〈0.01）.The oc-currence rates of anal margin edema ,anal itching ,urinary retention ,stool bleeding and incision bleeding of treatment group were all sig-nificantly lower than that of control group（ P〈0 .05）.Conclusion PPH is more effective than the Milligan-Morgan operation in the treatment of severe hemorrhoids ,which is worthy of recommendation in clinical treatment .
