Analysis on 589 cases of adverse events following immunization

目的分析东莞市2013年疑似预防接种异常反应（AdverseEventsFollowingImmunization，AEFI）的发生特征，为促进预防接种安全提供依据。方法通过AEFI监测系统，收集东莞市2013年AEF1个案数据，采用描述性方法进行分析。结果共收到个案589例，其中男女性别比为1．73：1；〈2岁占70．29％；6—8月份发生最多；共占38．53％；一般反应326例，占55．35％，主要以发热、局部红肿、局部硬结为主，涉及疫苗报告数居前3位的是百白破（无细胞）疫苗、Hib疫苗、23价肺炎疫苗；异常反应213例，占36．16％，主要以过敏性皮疹为主，涉及疫苗报告数居前五位的是麻风疫苗、水痘疫苗、百白破（无细胞）疫苗、Hib疫苗、脊灰减毒（猴肾细胞）疫苗；不良反应主要集中在接种后≤1d，占89．31％；所有AEF1个案均治愈或好转，未见有后遗症病例和死亡病例。结论AEFI常发生在〈2岁Jll童、棒种后≤1d：此为峨涮重点．

Objective To analyze the features of adverse events following immunization （AEFI）in Dongguan city,2013, and to provide basis for promoting immunization safety related to vaccines. Methods 2013 AEFI cases were collected through the national AEFI information system, reported from Dongguan city. Descriptive method was used in the study. Results 589 cases were collected. Among them, the sex ratio of male and female was 1.73 ： 1, target-age was〈2 year, accounted for 70.29%. 38.53% occurred from June to August. 362 cases were of common or minor reactions, accounted for 55.35%. The main symptoms were fever, local swelling and induration. The top three vaccines were： diphtheria-pertussis- tetanus （no cell） combined vaccine （DTap）, haemophilus influenza type B vaccine polysaccharide conjugated vaccine （Hib）, 23-valent pneumoceccal polysaccharide Vaccine （PPV23）. 213 cases were of rare serious vaccine reactions, accounted for 36. 16%, the main diagnosis were anaphylactic rash. The top five categories were measles and rubella combined attenuated live vaccine （MR）, varicella attenuated live vaccine （VarV）, DTap, Hib, poliomyelitis attenuated live vaccine （monkey kidney Cell）. 89.31% occurred ~〈 1 day after vaccination. All AEFIs were cured or getting better without sequela and death. Conclusions AEFIs often occurred in children aged 〈2 year, within 1 day after vaccination, this age group and period were the priority of AEFI surveillance.
