摘要
综述了延续性护理的概念及发展背景,总结了国内外延续性护理开展的现状及方式,并分析了我国开展延续性护理存在的问题及解决方法。
It reviewed the concept and development background of continuity of care.It summarized the status quo of development of continuity of care at home and in abroad,and analyzed problems in the development of continuity of care in China and its solutions.
