丁丙诺啡对臂丛麻醉患者的镇痛效果分析

Analgesic effect analysis of buprenorphine in brachial plexus anesthesia patients

目的 探讨丁丙诺啡对臂丛麻醉患者的镇痛效果。方法 选取本院2012年2～6月收治的60例臂丛神经阻滞下患者．将其按照抽签法随机分为两组，每组30例。对照组采用0．25％布比卡因与1％利多卡因联合麻醉。观察组使用丁丙诺啡麻醉，观察两组患者的麻醉镇痛效果。结果 经过麻醉后，对照组患者麻醉起效时间为（3．52±1．33）min、麻醉维持时间为（522．69±281．36）min，观察组分别为（4．19±1．36）min、（865．24±461．36）min，对照组患者疼痛视觉模拟评分为（1．36±0．52）分、观察组为（0．32±0．71）分；经过麻醉12h后，对照组评分为（3．65±1．68）分、观察组为（1．25±1．12）分；经过麻醉24h后，对照组评分为（2．39±0．98）分、观察组为（0．71±0．42）分。两组9、12、24h疼痛视觉模拟评分差异有统计学意义（P〈0．05）。结论 臂丛神经阻滞下患者采用丁丙诺啡麻醉镇痛的效果优良．麻醉时间持续较长，术后一定时间内仍然能够起到良好的镇痛效果，值得临床推广应用。

Objective To explore the analgesic effect of buprenorphine in brachial plexus anesthesia patients. Methods Sixty patients with brachial plexus block admitted into our hospital from February to June in 2012 were selected,they randomly divided into two groups according to the draw method,each group of 30 cases.In control group,O.25% bupivacaine and 1% lidocaine were jointly used for anesthetization,while in observation group,only buprenorphine was used. The analgesic effect in both groups was observed. Results In the control group,the average time of anesthesia taking effect was （3.52±1.33） min and average time of maintenance of anesthesia was （522.69±281.36） min,while in the observation group,they were （4.19±1.36） min and （865.24±461.36） min in turn.In the control group,the scores of VAS in brachial plexus block patients was （1.36±0.52） after 9 hours,（3.65±1.68） after 12 hours,and （2.39±0.98） after 24 hours, while in the observation group,they were （0.32±0.71）,（1.25±1.12）,and （0.71±0.42） respectively,which displayed statistical differences in these three matched time periods in both groups （P〈0.05）. Conclusion Application of buprenorphine for patients with brachial plexus block can obtain a great analgesic effect,last for a long time,and still take effect within a certain time after surgery,which is worthy of expansion in clinic.
