摘要
目的通过对3种脉冲多普勒方法测量正常胎儿左心室Tei指数的对比分析,探讨双源多普勒超声在评估胎儿左心功能方面的优势。资料与方法分别采用脉冲多普勒联合法、传统分开法及双源多普勒法测量57例孕20~38周胎儿的左心室Tel指数,并进行比较分析。结果脉冲多普勒联合法与传统分开法、双源多普勒法测量的a值(t=-26.49、24.62,P〈0.05)、b值(t=20.34、17.89,P〈0.05)、Tel指数比较(t=2.50、2.44.P〈0.05),差异均有统计学意义。脉冲多普勒联合法与传统分开法、双源多普勒法测量的a值(r=0.49、047,P〈0.05)、b值(r=0.46、0.40,P〈0.05)均呈低度正相关,而Tei指数间均无明显相关性(r=0-31、0.29,P〉0.05)。传统分开法与双源多普勒法测量a、b值及Tei指数之间均呈显著正相关(r=0.84、0.80、0.86,P〈0.05)。结论双源多普勒法测量胎儿左心室Tei指数更为科学可靠,可以保证频谱的质量与同步性,排除二尖瓣反流的干扰,在心律失常胎儿的左心功能评估中具有较大的潜在应用价值。
Purpose To explore the advantages of dual-source Doppler ultrasound in the assessment of fetal left ventricular function through the comparative analysis of three kinds of pulse Doppler methods to measure left ventricular Tei index in normal fetus. Materials and Methods Tei index of left ventricular in 57 fetus with gestational age from 20 to 38 weeks was measured using combined pulsed Doppler, traditional separate source Doppler and dual-source Doppler method respectively, and the results were comparatively analyzed. Results The differences were statistically significant tbr the a value (t=26.49 and 24.62, P〈0.05), b value (t=20.34 and 17.89, P〈0.05) and Tei index (t=2.50 and 2.44, P〈0.05) measured with the three different methods mentioned before. There were positive correlations of a value (r=0.49 and 0.47, P〈0.05) and b value (r=0.46 and 0.40, P〈0.05) measured between combined pulsed Doppler and traditional separate source Doppler as well as dual-source Doppler method, but no correlation of Tei index (r=0.31 and 0.29, P〉0.05). The a value, b value and Tei index showed positive correlations between traditional separate source Doppler and dual-source Doppler method (r=0.84, 0.80 and 0.86, P〈0.05). Conclusion It is more scientific and reliable to measure fetal left ventricular Tei index using dual-source Doppler examination, which is able to insure the quality and synchronization of the spectrum, exclude mitral regurgitation interference, with great application potential in the assessment of left ventricular function in fetal arrhythmias.
