摘要
目的观察纳米银外用敷料联合重组人表皮生长因子对烧伤创面的治疗效果。方法选择浅Ⅱ度及深Ⅱ度创面共96例,分别随机分为A组34例(纳米银敷料联合重组人表皮生长因子换药),B组32例(单纯纳米银敷料换药),C组30例(用碘伏凡士林纱布换药),于伤后7、10、15、20、25d分别对3组进行创面分泌物细菌培养及鉴定,全程观察记录创面愈合率及愈合时间,观察药物不良反应。结果浅Ⅱ度和深Ⅱ度创面细菌检出率,A组与B组比较差异均无统计学意义(P>0.05),C组明显高于A、B组,差异均有统计学意义(P<0.05)。伤后7d开始,无论浅Ⅱ度创面,还是深Ⅱ度创面,A、B组创面愈合率均较C组明显增大,差异均有统计学意义(P<0.05)。3组间创面平均愈合时间比较,A组最短,B组次之,C组最长,并且3组间的两两比较差异均有统计学意义(P<0.01)。3组均无过敏反应,A组有1例浅Ⅱ度创面患者换药后出现局部烧灼感,10min后症状自行缓解。结论纳米银敷料抗菌作用强,重组人表皮生长因子能促进组织修复,两者联合可以加速烧伤创面愈合。
Objective To investigate the curative effect of nanometer silver dressing combined with recombinant human epidermal growth factor on II degree burn wound. Methods Ninety-six cases of shallow II degree and deep II degree burn pa- tients were selected. They were divided into three groups randomly. Group A including 34 cases was treated by nanometer sil- ver dressing combined with recombinant human epidermal growth factor, and group B was treated by nanometer silver dressing only, and group C was treated by Iodine volts vaseline gauze. Bacteria of secretions in three groups were cultured and identified 7 days, 10 days, 15 days, 20 days, 25 days after injury. Born healing rate and time were observed and recorded. The adverse reactions of drug were observed. Results About bacterial detection rate of shallow II degree and deep II degree, group A and group B had no statistically significant difference (P〉0.05); group C was higher than group A and B, and had statistically sig- nificant difference (P〈0.05). From 7 days after injury, born healing rate of A group and group B increased significantly than that of the C group, the difference had statistical significance (P〈0.05). About born healing time, the shortest was group A, the secondry was group B, the longest was group C, and pairwise comparisons among the three groups had statistical signifi- cance (P〈0.01). The three groups had no allergic reaction. Local burning sensation appeared in one patient of degree light II in group A, and relieved voluntarily after 10 minutes. Conclusion Nanometer silver dressing has strong antibacterial effect. Recombinant human epidermal growth factor can promote tissue repair. Silver dressing combined with recombinant human epidermal growth factor can accelerate burn wounds to heal.
