胃镜引导腹腔镜手术治疗胃间质瘤22例

Gastroscopy-guided laparoscopy for gastric stromal tumors： a report of 22 cases

目的 探讨胃镜引导腹腔镜手术治疗胃间质瘤的应用价值.方法 回顾性分析我院2008年6月至2013年10月期间在胃镜引导下行腹腔镜手术治疗的22例胃间质瘤患者的临床资料.结果 22例患者在胃镜引导腹腔镜下成功切除胃间质瘤,肿瘤切除完整,边缘无残留.手术切口长约4～6 cm,平均5 cm,手术时间40～ 90 min,平均70 min,术后恢复顺利,未发生手术相关并发症.术后住院时间4～6 d,平均5.2d.随访2～48个月,平均26.5个月,未发生复发或转移.结论 胃镜引导腹腔镜手术能够切除单纯腹腔镜下难以定位的胃间质瘤,同时可以使切除范围更加精确,避免肿瘤残留及切除过多正常胃组织.

Objective To explore the value of gastroscopy-guided laparoscopy for gastric stromal tumors.Methods The clinical data of 22 patients with gastric stromal tumors performed gastroscopyguided laparoscopy from June 2008 to October 2013 in this hospital were analyzed retrospectively.All the gastric stromal tumors were successfully excised by gastroscopy-guided laparoscopy with clear margins.Results The length of surgical incision was about 4 ～ 6 cm,with an average of 5 cm.The mean operation time was 70 min.Postoperative recovery was smooth and no surgery-related complications occurred.The postoperative hospital stay was 4 ～ 6 d,with an average of 5.2 d.The follow-up period was 2 ～ 48 months,with an average of 26.5 months.No recurrence or metastasis happened.Conclusion Gastroscopy-guided laparoscopy can remove the gastric stromal tumors which cannot be fixed by simple laparoscopy.In the treatment of gastroscopy-guided laparoscopy,the resection range can be more precise,and residual tumor can be avoided without excessive resection of normal gastric tissues.
