摘要
目的 分析厄贝沙坦氢氯噻嗪联合美托洛尔治疗老年重症心力衰竭的疗效.方法 选取2011年10月-2013年10月我科收治的老年重症心力衰竭患者86例,将其随机分为对照组和治疗组,各43例.对照组采用临床急诊内科常规强心、利尿剂,硝普钠静脉微量注射泵注射等治疗;治疗组在对照组治疗的基础上,采用厄贝沙坦氢氯噻嗪联合美托洛尔治疗.结果 治疗组患者心脏功能复常时间、心电图表现复常时间及临床用药方案实施时间均短于对照组（P＜0.05）;治疗后治疗组患者B型脑钠肽（BNP）低于对照组,左室射血分数（LVEF）高于对照组（P＜0.05）;经治疗治疗组患者总有效率为90.7％ （39/43）,高于对照组的69.8％ （30/43） （P＜0.05）;治疗组患者不良反应发生率和心力衰竭复发率均低于对照组（P＜0.05）.结论 厄贝沙坦氢氯噻嗪联合美托洛尔治疗老年重症心力衰竭效果显著,能改善患者心功能,药物不良反应少,病情复发率低,安全性高.
Objective To analyze the effect of Irbesartan and Hydrochlorothiazide combined with Metoprolol on elderly patients with severe heart failure. Methods 86 patients with severe heart failure admitted to our hospital from October 2011 to October 2013 were randomly divided into groups control and treatment, each of 43 cases. Control group was given conventional treatment, treatment group was given Irbesartan and Hydrochlorothiazide combined with Metoprolol on basis of control group. Resuits The time of cardiac function normalization, ECG normalization and clinical drug program implementation in treatment group were shorter than those of control group （P 〈 0. 05） . After treatment, the BNP in treatment group was lower than that of control group, LVEF higher than that of control group （P 〈0. 05） ; the total efficacy of treatment group was 90. 7% （39/43）, higher than that of control group of 69. 8% （30/43） （ P 〈 0. 05 ） ; the incidence of adverse reactions and recurrence rate of heart failure were lower than those of control group （P 〈 0. 05）. Conclusion Irbesartan and Hydrochlorothiazide combined with Metoprolol in treating elderly patients with severe heart failure can improve cardiac function, being with few adverse reaction, recurrence rate and high safety.
