摘要 目的通过对我国医院分院建设现状分析,为我国医院分院设立与建设提供实证依据。方法检索我国部属部管医院、2013年度复旦大学最佳医院及美国梅奥综合医学中心、日本德洲会医疗集团、台湾长庚医院官方网站及中国生物医学文献光盘数据库(CBMdisc)、中国期刊全文数据库(CNKI)数据库、维普(VIP)、万方、国际性综合生物医学信息书目数据库(Medline)。提取有关分院设立的资金、人员、经营模式等数据并进行描述性分析。结果①本研究调查的地方医院共73家,其中36家开设了分院(45%);②北部和东部地区在开设分院的医院数量、开设分院数量分别位居前2位;③分院等级主要为二级医院,有25家(32%),主要分布于东部和西部,分别为12家(48%)、7家(28%);④综合型分院有31家(40%);专科型分院有16家(20%);⑤本部外派结合社会招聘,也开展内部选拔和外部引进,16家(20%)分院由本部派出结合社会招聘,派出方式包括定期不定期地派遣、常驻等;⑥19家(24%)分院采取托管;⑦地方政府制定的有关医疗体制改革的发展规划是分院建设的政策依据。结论目前所开办的各种类型分院托管是现有主要运营模式,分院人员组成方式灵活多样,以本部派出结合社会招聘为主,根据当地疾病诊治需求进行科室设置,以二级医院为主,将综合和特色专科相结合。应深入探索有效实施途径,积极争取政策支持,大力开展分院建设。 Objective To analyze the current situation of hospital branches in China, and thus, to provide a reference for es- tablishment and construction of hospital branches. Methods Data were retrieved from multiple sources including the official website of affiliated hospital in China, the 2013 annual best hospitals of Fudan University, the Mayo Clinic, the Japan's Texas Medical Group, the Chang-Gung Memorial Hospital in Taiwan, the Chinese biomedical literature CD-rom database (CBMdisc), the China journal full-text database ( CNKI), the VIP database, the Wanfang database and Medline. Extract data about funds, personnel, business pattern of hospital branches and descriptive analysis was used for this survey. Results (1)Seventy-three local hospitals was included in this study. Of these hospitals,36 hospitals(45% ) had branches. (2)Northern and eastern area ranked top two in the number of hospitals establishing bran- ches and the number of branches, respectively. (3)The majority of hospital branch are second-class hospitals, in which 25 second-class hospitals (32%) were locate in the eastern and southern area 12 (48 % ) in the eastern area and 7 (28 % ) in the southern area, respec- tively). (4)There were 31 (40%) comprehensive hospital branches and 16 ( 20% ) specialist hospital branches. (5)16 ( 20% ) hospital branches were department external assignments combined with social recruitment. The assignments also included regularly or irregularly sending and resident, and so on. (6)The 24% of the branches were taken trusteeship. (7) The development planning about healthcare re- form set by local government is the policy for hospital branch's construction. Conclusion The trusteeship is the major operation mode for the current hospital branches. Branch staffs are organized with flexible manner: department external assignments combined with social recruitment. The departments were set-up according to local demands. The most branches are most secondary-class hospitals combined with comprehensive and characteristic specialized subject. We should further explore the effective implementation ways, strive the policy support and vigorously carry out the constructions.
