炔雌醇环丙孕酮联合二甲双胍治疗多囊卵巢综合征型不孕症的效果以及并发症观察

Observation of the effect and complications of Diane-35 combined with metformin in the treatment of polycystic ovary syndrome infertility

目的观察炔雌醇环丙孕酮联合二甲双胍治疗多囊卵巢综合征型不孕症的临床疗效以及并发症的发生率。方法选取2010年10月到2012年10月期间治疗的多囊卵巢综合征型不孕症患者138例作为研究对象。随机分为实验组和对照组,实验组采取炔雌醇环丙孕酮联合二甲双胍治疗,对照组采取口服炔雌醇环丙孕酮治疗。观察两组治疗前后的体重、体质指数(BMI)、促黄体生成素(LH)、促卵泡刺激素(FSH)、睾酮(T)、雌二醇(E2)、空腹胰岛素(IN)、排卵率、月经恢复率、妊娠率以及并发症的发生率。结果两组治疗前后的体重、BMI、LH、FSH、T、E2以及IN差异具有统计学意义(P<0.05),两组治疗前比较无明显差异(P>0.05),两组治疗后比较除了FSH外,其余指标实验组均优于对照组,差异均具有统计学意义(P<0.05)。实验组的月经恢复率(97.14%)、排卵率(87.14%)、妊娠率(34.29%)明显高于对照组月经恢复率(67.64%)、排卵率(61.76%)、妊娠率(19.12%),实验组的并发症发生率(4.29%)明显低于对照组(22.06%),两组比较差异均具有统计学意义(P<0.05)。结论炔雌醇环丙孕酮联合二甲双胍治疗多囊卵巢综合征型不孕症疗效显著,并发症低,具有一定的临床价值。

Objective To observe the clinical efficacy and incidence of complications of Diane-35 combined with metformin in the treat-ment of infertility caused by polycystic ovary syndrome（ PCOS）. Methods 138 infertile patients with PCOS treated in our hospital from October 2010 to October 2012 were selected as the research subject,and were randomly divided into experimental group and control group. The experimen-tal group took Diane-35 combined with metformin,the control group received oral administration with Diane-35 treatment. Observed before and after treatment,body weight,BMI,LH,FSH,T,E2,IN,ovulation rate menstrual recovery rate,pregnancy rate and incidence of complications. Results There were significant differences in body weight,body mass index（ BMI）,luteinizing hormone（ LH）,follicle-stimulating hormone （FSH）,testosterone,Estradiol and fasting insulin levels（allP 〈0.05）after treatment in comparison with those before treatment in both groups. Although no significant difference in these indexes（ P 〉0. 05）was found between two groups before treatment;all the indexes excepting FSH af-ter treatment showed statistically significant differences（all P 〈0. 05）between two groups after treatment. The menstrual recovery rate（97. 14%）,ovulation rate（87. 14%）and pregnancy rate（34. 29%）of experimental group were significantly higher than those of the control group（ 67. 64%,61. 76%,and 19. 12%,respectively）;and the incidence of complications in experimental group（4. 29%）was significantly lower than the control group（22. 06%）,the differences between the two groups were statistically significant（all P 〈0. 05）. Conclusion The admin-istration of Diane-35 combined with metformin has obvious curative effect with low complication rate in the treatment of PCOS infertility.
