乙型肝炎病毒感染对体外受精-胚胎移植临床结局的影响

Effect of B hepatitis on outcome of in vitro fertilization and embryo transfer

目的探讨乙型肝炎病毒(HBV)感染对体外受精-胚胎移植(IVF-ET)结局的影响。方法对珠海市妇幼保健院生殖中心2011年1月至2012年12月接受IVF-ET治疗的HBV感染不育病例178例,共178个新鲜周期回顾性分析。女方HBV感染共82个周期为A组,男方HBV感染共96个周期为B组,无HBV感染共682个周期为C组(对照组)。分析和比较3组在年龄、不育年限、Gn天数、Gn剂量、血清基础卵泡刺激素(FSH)、黄体生成素(LH)、雌激素(E2)水平、HCG日子宫内膜厚度、获卵数、受精率、卵裂率、可移植胚胎数、种植率、临床妊娠率、流产率的差异。结果 3组在年龄、不育年限、Gn天数、Gn剂量、血清基础FSH、LH、E2水平、HCG日子宫内膜厚度、获卵数、受精率、卵裂率、可移植胚胎数、种植率、临床妊娠率间相互比较,差异无统计学意义(P均>0.05)。B组的流产率为10.45%,显著高于C组,差异有统计学意义(P<0.05)。结论 HBV感染者接受IVF-ET治疗不影响临床结局,但男方HBV感染时可能增加流产风险。

Objective To analyze the impact of the hepatitis B virus（HBV） on the outcome of in vitro fertilization and embryo transfer （IVF-ET）. Methods 178 fresh cycles from infertile patients with B hepatitis who underwent IVF-ET at Zhu hai Maternal and Children＆#39;s Hospital and Health Institute between January, 2011-December, 2012, were retrospectively analyzed. The patients were divided into group A （women with hepatitis B virus infection, n=82）, group B （men with hepatitis B virus infection, n=96） and group C as a controlling group（without hepatitis B virus infection, n=682）. The difference of pregnancy outcomes was compared and analyzed among the different groups. The difference of pregnancy outcomes was compared and analyzed among the different groups. Results No statistically significant difference was found in age, duration of infertility, amount of Gn and period of ovarian stimulation, level of basic FSH, LH, E2, endometrial thickness on the day of injecting HCG, number of retrieved oocytes, fertilization rate, cleavage rate, embryos could be transferred, implantation rate, clinical pregnancy rate among the three groups（P〉0.05）. The abortion rate in group B was 10.45%, higher than in group A or group C, the difference was statistically significant （P〈0.05）. Conclusion There might be no adverse effects of B hepatitis on the outcome of IVF-ET,but the abortion rate might be higher in men with hepatitis B virus infection.
