依托芬那酯凝胶对比双氯芬酸二乙胺乳胶治疗髌股关节骨关节炎的临床观察

Clinical Observation of Etofenamate Gel vs. Diclofcnac Diethylamine Emulsion in the Treatment of Patellofemoral Osteoarthritis

目的:观察依托芬那酯凝胶对比双氯芬酸二乙胺乳胶治疗髌股关节骨关节炎的临床疗效和安全性。方法:94例髌股关节骨关节炎患者按随机数字表法均分为试验组和对照组。试验组患者给予10%依托芬那酯凝胶,根据疼痛部位大小,每次涂3 cm药物在局部并用手轻按摩,tid;对照组患者给予1%双氯芬酸二乙胺乳胶,根据疼痛部位大小,每次涂3 cm药物在局部并用手轻按摩,tid。两组患者患处不覆盖敷料或绷带,疗程均为7 d。观察两组患者的临床疗效、20 m步行痛评分、西方安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分、关节触痛评分及不良反应发生情况。结果:治疗后试验组患者总有效率显著高于对照组,两组比较差异有统计学意义(P<0.05)。治疗前两组患者20 m步行痛评分、WOMAC评分、关节触痛评分比较,差异无统计学意义(P>0.05);治疗后两组患者20 m步行痛评分、WOMAC评分、关节触痛评分均显著低于同组治疗前,且试验组低于对照组,差异有统计学意义(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:依托芬那酯凝胶治疗髌股关节骨关节炎疗效优于双氯芬酸二乙胺乳胶,安全性相当。

OBJECTIVE： To observe clinical efficacy and safety of Etofenamate gel vs. Diclofcnac diethylamine emulsion in the treatment of patellofemoral osteoarthritis （PFOA）. METHODS： 94 patients with PFOA were randomized into trial group and control group. Trial group was given 10% Etofenamate gel with 3 cm length according to the size of pain site 3 times a day. Con trol group was given 1% Diclofenac diethylamine emulsion with 3 cm length according to the size of pain site 3 times a day. Affected parts were not covered by a dressing or bandage for 7 days. Clinical efficacy, 20 m walking pain score, WOMAC score, joint tenderness pain and the incidence of ADR were observed in 2 groups. RESULTS： Total effective rate of trial group was significant ly higher than that of control group; there was statistical significance （P〈0.05）. 20 m walking pain score, WOMAC score and joint tenderness pain had no statistical significance between 2 groups before treatment （P〈0.05）. 20 m walking pain score, WOMAC score and joint tenderness pain of 2 groups after treatment were significantly lower than before; and those of trial group was significantly lower than those of control group; there was statistical significance （P〈0.05）. The incidence of ADR had no sta tistical significance between 2 groups （P〉0.05）. CONCLUSIONS： Etofenamate gel diclofcnac diethylamine emulsion has signifi cant clinical efficacy and good safety in the treatment of PFOA, but Etofenamate gel has better therapentic efficacy.
