超声心动图诊断亚急性感染性心内膜炎的效果分析 被引量:7

Effect of echocardiography on diagnosis of subacute infective endocarditis

摘要 目的分析超声心动图对亚急性感染性心内膜炎的临床诊断价值,为亚急性感染性心内膜炎的诊断提供参考依据。方法选择医院2008年5月-2012年2月收治的83例亚急性感染性心内膜炎患者,对其进行超声心动图检查和手术病理检查,对检查结果进行对比分析。结果83例患者中用超声心动图检查出赘生物的有80例,阳性率为96.4%,其中检出赘生物位于二尖瓣的患者有21例占26.25%、三尖瓣15例占18.75%、主动脉瓣17例占21.25%、肺动脉瓣9例占11.25%、二尖瓣和主动脉瓣13例占16.25%、人工瓣5例占6.25%;超声心动图检出的赘生物〈2mm的14例占17.50%、2~15mm的55例占68.85%、〉15mm的11例占13.75%;超声心动图显示赘生物数量为1个的21例占26.25%、2个的45例占56.25%、≥3个的14例占17.50%;超声心动图检查结果与手术诊断结果比较,差异无统计学意义。结论超声心动图能够快速显示出赘生物的位置、大小和数量,能较准确地诊断亚急性感染性心内膜炎,值得在临床推广应用。 OBJECTIVE To analyze the clinical value of echocardiography in diagnosis of subacute infective endocar- ditis so as to provide guidance for the diagnosis of subacute infective endocarditis. METHODS A total of 83 patients with subacute infective endocarditis, who were treated in the hospital from May 2008 to Feb 2012, were enrolled in the study, then the echocardiography and pathological examination were performed for all the subjects, the re- suits were analyzed. RESULTS The neoplasms were detected in 80 of 83 patients with the use of echocardiography, and the positive rate was 96.4%. Among the 80 patients, there were 21 (26.25%) cases with the neoplasms loca- ting in mitral valve, 15 (18.75%) cases with the neoplasms locating in tricuspid valve, 17 (21.25%) cases with the neoplasms locating in aortic valve, 9 (11.25%) cases with the neopasms locating in pulmonary valve, 13 (16.25%) cases with the neoplasms locating in the mitral valve and aortic valve, and 5 (6.25%) cases with the neoplasms locating in artificial valve. Totally 14 (17.50%) cases with the size of neoplasms less than 2mm, 55 (68.85%) cases with the size ranging from 2 to 15mm, and 11 (13.75%) cases with the size more than 15ram were detected with the use of echocardiography. The echocardiography revealed 21 (26.25%) cases with 1 neo- plasm, 45 (56.25%) cases with 2 neoplasms, and 14 (17.50%) cases with more than 3 neoplasms; as compared with the result of the echocardiography and the surgical diagnosis result, the difference was not significant. CONCLUSION The echocardiography can rapidly reveal the location, size, and number of the neoplasms and accu- rately diagnose the subacute infective endocarditis, and it is worthy to be promoted in the hospital.
