细菌耐药监测结果与抗生素合理使用 被引量:4

摘要 目的通过对本院抗菌药物临床应用频度及本地区分布、细菌耐药情况的分析,以促进临床抗菌药物的合理使用。方法采用限定日剂量(DDD)法分析本院各种抗菌药物的使用情况,回顾性调查本院2012年检出的细菌及其药物敏感试验结果。细菌培养及药敏检测采用外购药敏试纸,按照《全国临床检验操作规程》第3版要求进行常规鉴定。根据细菌耐药监测数据,分析病原菌对抗菌药物的耐药情况。结果细菌培养共收集有效标本1 096份,分离鉴定出致病菌267株,阳性率24.4%,其中革兰阳性菌占30.2%,革兰阴性菌占69.8%,临床分离出病原菌主要有大肠埃希菌、葡萄球菌属、铜绿假单胞菌等,其中大肠埃希菌检出最多,多数致病菌对常用抗生素耐药率较高。结论对细菌进行耐药监测及分析,可为临床经验合理用药提供有力的技术支持,对临床合理选择抗菌药物具有重要价值。 Objective To investigate the clinical application frequency and local distribution of antibacterial drugs as well as drug resistance of bacteria in the hospital, so as to promote the rational application of clinical antimierobial drugs. Methods The application of antibiotics in bacteria in the hospital was analyzed by the method of Defined daily dose (DDD). The drug sensitive test of the bacterial culture and drug sensitivity test in the hospital were retrospectively reviewed from 2012 annual survey. The bacterial culture and drug susceptibility test were done by using outsourcing drug sensitivity test, in accordance with the "national clinical laboratory operation rules" version third requirements for routine identification. The antimicrobial resistance of pathogens was analyze according to the bacterial drug resistance monitoring data. Results A total of 1 096 effective samples were collected. 267 strains of bacteria were isolated and identified, and the positive rate was 24.4%, the gram positive bacteria accounted for 30.2%, gram negative bacteria accounted for 69.8%. The pathogens isolated from clinical were Escherichia coli, Staphylococcus aureus, Pseudomonas aeruginosa, Escherichia coli. Escherichia coli was the most common one, and most of the pathogenic bacteria resistance to commonly used antibiotics was relatively high. Conclusion The bacterial resistance monitoring and analysis of bacteria can provide technical support for the rational clinical drugs application, and it has important value for the rational application of antimicrobial agents in clinic.
