心血管急症救治(7)心肺复苏推荐标准的循证依据与更迭(续6) 被引量：7

摘要 最新心肺复苏推荐标准简化了基本生命支持(BLS)流程,使"A-B-C"(气道-呼吸-胸外按压)顺序更改为"C-A-B"(胸外按压-气道-呼吸);认同单纯CPR的作用;提出确保高质量CPR。在高级心血管生命支持中,增加了新的"生存链"心脏骤停后的综合救治的第五个环节,其主要目标:①维护及优化自主循环恢复(ROSC)后患者心肺功能和重要器官的灌注;②转运至适合的医院或综合心脏骤停后救治的监护病房;③鉴别和对急性冠状动脉综合征(ACS)患者采取干预性治疗;优化体温控制治疗,有益神经功能恢复。自2010指南发布后又不断有不同的研究结论发表:①新推荐按压幅度并未带来理想的效果;②最有效的按压部位可能是胸骨剑突联合部;③在心脏骤停复苏低温目标管理中,低温治疗的目标温度33℃组未必较36℃组更佳;④有关心脏骤停及复苏后损伤机制的研究,相关流行病学分析,低温治疗与微循环功能障碍,以及各类辅助复苏方法的应用,对更新推荐标准提供了新的循证依据。
