原发抑郁症与脑卒中后抑郁症患者的海马形状分析 被引量:5

THE ANALYSIS OF HIPPOCAMPUS SHAPE IN PATIENTS WITH PRIMARY DEPRESSION AND POSTSTROKE DEPRESSION

摘要 目的探讨原发抑郁症患者与脑卒中后抑郁症患者的海马形状改变模式的区别。方法选取22例脑卒中后抑郁症(PSD)患者、15例原发抑郁症(PD)患者及29例正常对照者,使用计算机自动分割和顶点分析技术,获得三组的校正后海马体积和海马表面模型,分别进行体积和形状的组间比较。结果体积比较显示PD组与对照组之间的左右侧海马体积均差异无统计学意义(左侧P=0.90,右侧P=0.19),PSD组与对照组之间的左右侧海马体积也差异无统计学意义(左侧P=0.12,右侧P=0.25);形状分析显示PD和PSD组的双侧海马局部区域较对照组存在萎缩,PD组以左侧海马头部(CA1区)明显,PSD组以左侧海马体部(CA3区)明显。结论 PD与PSD患者的海马存在容易被常规体积测量方法所忽略的形状改变,其形状改变模式的区别可能有助于疾病的早期诊断、鉴别和探讨疾病间的潜在联系。 Objective To study the difference of hippoeampus morphology of patients with primary depression and post - stroke depression. Methods The volume after correction and the surface mold of the hippocampus of 22 PSD patients, 15 PD patients and 29 healthy controls were attained and compared through au- to- segmentation and vertex analysis technology. Results Volume compare showed no statistical difference of the bilateral hippocampus volumes between the PD patients and health controls(left P = 0.90, right P = 0.19), and no statistical difference between the PSD patients and health controls(left P = 0.12, right P = 0.25). But shape analysis showed that local atrophy in hippocampus was found in MS and PSD patients compared with health group, mainly at left hippocampus body (CA3) of PSD patients, and the left hippocampus head (CA1) in PD patients. Conclusions The hippoeampus of PD and PSD patients has shape changes that may not be found with conventional volume measure. The difference of hippocampus shape changing mode may contribute to the early diagnosis, distinguishing, and searching the potential connections among diseases.
